A social wellness agency in New York City is actively seeking a new Licensed Practical Nurse (LPN) for a great opportunity with their growing Assertive Community Treatment (ACT) team. In this role, the Licensed Practical Nurse (LPN) will be responsible for providing management and administration of medications for persons with severe and persistent mental illness.
Responsibilities:

The Licensed Practical Nurse (LPN) will:

  • Work with clients to develop an understanding of their symptoms and medications
  • Monitor for side effects
  • Participate in hospital discharge planning meetings and assist in the follow up of medical treatment post hospitalization or ER visits
  • Acquire discharge paperwork from hospitals and provide copies to team leader, prescribers, and program assistants
  • Maintain compliance with incident reporting guidelines
  • Report all incidents in a timely manor
  • Provide education to clients and staff regarding standards of medical care
  • Provide linkages to appropriate medical care providers when applicable
  • Perform other duties, as needed
Qualifications:
  • Graduate of an accredited Practical Nursing program
  • NYS Licensed Practical Nurse (LPN)
  • Proficiency in Nursing and Medication Administration
  • Knowledge of Treatment and Rehabilitation methods for persons with severe and persistent mental illness
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • 1+ year of experience in a Treatment and/or Rehabilitation setting
