Description:The Stroke Coordinator, reporting to the Vice President of Patient Care and Chief Nursing Officer, plays a crucial role in supporting the administrative and clinical aspects of the hospital’s Stroke Center. Responsibilities include maintaining practice standards for stroke patient care, collaborating with healthcare providers, patients, families, and the community, and ensuring compliance with regulations and accreditation requirements. The Stroke Coordinator reports to the Administrative Director and/or Nurse Manager. Responsibilities: Provide administrative support for the hospital-wide stroke program, including Primary Stroke Center certification. Serve as the primary liaison with the Connecticut Department of Public Health for certification purposes. Implement a medical record review process, conducting concurrent and retrospective reviews to abstract clinical data on stroke patients. Ensure timely feedback to clinicians for accurate and complete medical records. Maintain the “Get With the Guidelines” AHA database. Conduct day-to-day chart reviews for stroke patients and coordinate multidisciplinary services. Meet with families of newly diagnosed stroke patients. Develop, review, update, and distribute patient/family educational materials. Collaborate with the stroke team to create an annual education plan, prepare materials for outreach events, screenings, and lectures, and promote stroke awareness within the local community. Maintain records of community education to demonstrate Stroke Center Certification compliance. Act as a liaison between prevention personnel, pre-hospital staff, EMS, and rehabilitation facilities. Provide internal and external consultation on stroke-related efforts and facilitate stroke prevention community outreach. Participate in state-wide stroke meetings and conferences, coordinating with other Connecticut hospitals. Perform other assigned duties.Additional Skills & Qualifications:Requirements: Current Connecticut RN license and a minimum of 5 years of clinical experience. Working knowledge of CT DPH certification requirements and Joint Commission standards related to stroke patient care. Understanding of cerebrovascular disease pathophysiology and medical record documentation tools. Effective interpersonal, communication, time management, and analytical skills.
