General Summary:

Provide coordination of and linkage to services, advocacy, and direct services as needed to an individual and/or family for the purpose of assisting persons with their overall life management. To provide assessment of clients admitted to psychiatric hospitals and act as a Liaison for the purpose of assisting persons and their families with the development of clinically appropriate discharge plans and post hospital treatment

Essential Responsibilities:

  • Facilitate, review and coordinate referrals to/from instate/out of state hospitals, prison systems, and other organizations for adults needing Thrive Behavioral Health services to include the mobilization of support networks and intervention with significant others as appropriate.
  • Represent Thrive Behavioral Health at treatment team meetings at various hospitals and community settings;
  • Coordinate and monitor flow of civil certification process.
  • Coordinate referrals to appropriate Outpatient Mental Health and Substance Abuse Programs as needed.
  • Maintain case load of clients to provide inpatient liaison services and linkage with financial resources and insurance/CMAP.
  • Present verbal and written information on clients who have been hospitalized.
  • Maintain and verify hospitalization statistics.
  • Participate in designated meetings including meetings at psychiatric hospitals.
  • Development and implementation of treatment plans and documentation of services consistent with Thrive standards.
  • Complete clinical documentation in accordance with Thrive standards.
  • Provide crisis intervention.
  • Coordinate services within Thrive as needed.
  • Coordinate and monitor medical, dental, and psychiatric services, and interface with outside agencies as needed.
  • Provide daily case management to designated clients in the community.
  • Assist clients with locating and maintaining employment.
  • Provide individual, family or group counseling.
  • Monitor signs and symptoms of client's psychiatric functioning, monitor medication regimen and assist in community networking and education.

Additional Responsibilities:

  • Attend all agency/department required training and meetings within the prescribed period.
  • Attend all Safety Training programs and be able to describe his/her responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.

Education, Experience and Competencies:

Bachelor's Degree in social work, psychology or related human service field required. Two years of

experience in case management working with psychiatrically disabled clients required.

Certifications, Licenses Requirements:

Valid driver's license required. Case Management or Community Support Certification required.
