Job Overview:

The RWJ Rahway Community Health Worker (Navigator) will ensure patient access to healthcare and community services by interacting with Rahway s patients to address barriers to care, provide education and assist in the coordination of care in order to maintain a healthy lifestyle and meet identified health goals.

Qualifications:

Required:

* High school graduate or equivalent required

* Basic computer skills required

* Valid driver s license required

* Successful completion of a Community Health Worker formal training program is expected upon hire

Preferred:

* Electronic medical record (EMR) experience preferred

* Knowledge of medical terminology preferred

Scheduling Requirements (non-exempt positions remove when posting):

* 8:00am/8:30am to 4:30pm/5:00pm

* Monday-Friday

* Full-Time

Essential Functions:

Works with hospital care teams to identify, evaluate, and ensure timely patient access to appropriate health resources and services. Navigator will assess patient health related and socio-economic needs using Epic tools, including but not limited to, the SDOH screening survey

* Meet and establish trust with patients in the hospital setting including the ED, Transitions of Care clinic, hospital units and/or other setting where patients are located

* Assess and promptly connect patients with clinical services, across the RWJBH system of providers, RWJ Rahway s in-house TOC Clinic and/or local FQHC s as required. Provide social and community resources to patients as needs are identified

* Assist patients with resources needed to understand their health condition(s), access screenings, and develop strategies to improve their health and well-being

* Assist patients with resources to recognize identified social issues; substance abuse, food insecurity, transportation, childcare services, housing and homelessness and any other issues that are impacting the members lives

* Assist management in identifying and evaluating process and outcome measures to examine program success and future program improvement

* Document patient information no later than 24 hours after patient contact

* Maintain knowledge of, and relationships with, community-based organizations. Keep current a directory of regional/local referral sources

* Exhibit excellent working relationships with care delivery team, not limited to; Case Management, Community Health, hospital staff and community organizations

* Participation in interdisciplinary rounds, department meetings, safety huddles and other meetings as required

* Perform daily data entry. Collect and report data to hospital leadership as requested
