Job Status:Full Time

Reports To:Director of Admissions and Compliance

Position Supervises: N/A

Work Schedule:Primarily day hours, evening work may be required. Flexibility in scheduling.

POSITION SUMMARY

Provide a warm and welcoming admission and intake experience for new and potential participants. Complete initial screenings and assessments and assist participants in overcoming barriers to accessing treatment.

ESSENTIAL FUNCTIONS STATEMENT(S)

Primary Duties

  • Answer phone and email inquiries about enrollment in trauma grant services.
  • Provide initial engagement of potential participants to ensure referrals are appropriate for the program.
  • Complete required screenings and assessments and accurately record data for grant-reporting requirements.
  • Welcome new participants to the program and educate participants about the services offered.
  • Work with new participants to identify barriers to accessing treatment.
  • Provide care management to reduce barriers and increase retention in services.
  • Assist participants in scheduling appointments.
  • Maintain a working relationship and knowledge of available community resources.
POSITIONS QUALIFICATIONS

SKILLS & ABILITIES

Education & Experience: Bachelor's degree in social work, social services or other human services related field. A minimum of two years of experience or an equivalent combination of education and experience. Excellent interpersonal skills and coordinative abilities required.

Skills:
  • Excellent oral and written communication skills.
  • Organized and detail oriented.
  • Knowledge of Excel.

PHYSICAL DEMANDS

A. Ability to stand, walk, sit, manually manipulate, reach outward and above shoulders, speak, climb, crawl, squat, kneel, and bend on a daily basis.

B. Capability to lift, carry, push or pull up to 25lbs on a daily basis.
