Description:
Manager, Clinical Pharmacy - SPHP
Reports to the SPHP Director of Pharmacy Services, the clinical pharmacy manager is responsible for developing, monitoring, evaluating and coordinating clinical pharmacy services.
The clinical pharmacy manager:
1. Develops/updates and implements policies, procedures, process guidelines, and protocols to ensure the provision of safe, effective and economical pharmacotherapy for all patients in the hospital
2. Ensures compliance with clinical practice standards as well as Joint Commission and applicable federal, state and local laws, regulatory standards.
3. Oversees Pharmacy and Therapeutic Committee and actively contributes to other patient care/clinical/safety committees.
4. Is responsible for the direct supervision/support of clinical pharmacy staff
5. Responsible for development and implementation of quality improvement initiatives, fiscal controls, management of capital resources, formulary management, and risk management functions for the clinical pharmacy program.
6. Oversees and actively supports the Pharmacy Residency Program.
PREREQUISITES
1) Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education.
2) Current license and registration as a pharmacist in the state of New York.
3) 5 to 7 years of progressively more responsible hospital related work experience including personnel oversight.
4) Must qualify as a PGY1 Pharmacy Residency Program Director (RPD) (Standard 4) (May serve as RPD)
5) BCPS or equivalent BPS certification, pharmacy residency and/or leadership experience preferred and as required for RPD Qualifications.
6) Experience within settings that provide a depth of clinical, medication distribution, and management needed for oversight.
7) Skills required:
a. Analytical skills necessary to develop, implement, and administer policies, procedures, analyze financial and clinical data and prepare clear, concise reports that identify opportunities and propose appropriate action plans.
b. Interpersonal skills in order to coach mentor and effectively lead assigned personnel, facilitate/organize/direct the work of others and to maintain positive relationships among medical staff, hospital managers, executives and staff.
c. Must possess proven skills in written/verbal communications, problem solving, independent problem solving, decision-making abilities, creativity, prioritization/delegation, and necessary computer-related skill.
d. Knowledge of the principles, standards, and technology applicable to pharmaceutical services and to medication safety
e. Knowledge of the principles and practices of continuous process and performance improvement.
f. Knowledge of the NYS Pharmacy laws and regulations.
g. Ability to express information effectively in verbal and written communication and through group/business presentations.

PRINCIPLE DUTIES AND RESPONSIBILITIES
SERVICE:
1) Clinical Pharmacy Services
a. Coordinates clinical pharmacy program activities as directed by the Systems Pharmacy Director
b. Coordinates a documentation system which measures cost-savings/cost-avoidance activities of the clinical pharmacy program.
c. Coordinates departmental programs which monitor and support appropriate drug use outcomes.
i. Therapeutic drug monitoring

iii. Pharmacy provided medication reconciliation services
iv. Drug Information
v. Antimicrobial Stewardship
vi. Pain Management/Opioid Stewardship
vii. Other on-going programs
d. Oversees the Clinical Pharmacist schedules, and On-call program
e. Provides oral and written drug information to health-care professionals in a timely manner and appropriate to their levels of skill/training.
f. Provides feedback (verbal and written) to the department regarding appropriate therapy and adverse drug reaction reviews.
g. Coordinates clinical practice and Trinity Health clinical initiatives within St. Peter's Health Partners.
h. Provide oversight/guidance of Antimicrobial Stewardship Program
i. Assist director in development and maintenance of new programs and/or roles to meet evolving needs of the department
j. Lead and develop clinical programs, especially in specialty areas such as but not limited to: Meds to Beds
2) Formulary Management
a. Coordinates the SPHP Pharmacy and Therapeutics Committee and serves as a conduit for Trinity Health P&T outcomes.
i. Serves as P&T committee secretary and maintains all related records as required
ii. Communicates outcomes to the SPHP Medical Executive Committees & Pharmacy staff
b. Maintains the SPHP formulary.
c. Coordinates the monitoring of restricted and non-formulary drug use across SPHP
d. Identifies trends in drug use to assure ongoing maintenance of a drug formulary reflective of current practices
e. Identifies and communicates conservation strategies and/or therapeutic alternatives or for medication shortages effecting SPHP.
f. Provides regular feedback to Trinity Health Pharmacy and Therapeutics Committee.
3) Therapeutic Guidelines/Order Sets
a. Collaborates and coordinates the appropriate use of medications proposed for electronic order sets.
4) Educational Pharmacy Services
a. SPH PGY1 Residency Program
1. May serve as Residency Program Director
2. Member of the Residency Advisory Committee
3. Oversees and conducts the hiring, performance management and termination process of the residents.
a. Participates in recruitment events: ACPHS Career Day, ASHP Clinical Midyear meeting, and others as required.
4. Maintains current policies and procedures required to administer the program.
5. Oversees the scheduling the residents and tracks compliance with duty hours.
6. Coordinates the gathering and staging of major research projects for the residency program and serves as a mentor for preceptors of such projects.
7. Oversees the development of residency, APPE and IPPE preceptors.
8. Coordinates and maintains all records required for the CMS cost report.
ii. Serves as primary preceptor for the required Pharmacy Practice Management learning experience.

QUALITY
1) Medication Safety Program
a. Coordinates the Medication Safety Sub-committees
i. Serves as committee secretary and maintains all related records
ii. Communicates outcomes to the appropriate medical or quality committees/leadership.
b. Reviews and analyzes SPHP reported medication events.
c. Coordinates the Adverse Drug Event reporting system within SPHP
d. Serves as the MIDAS Coordinator for the Pharmacy.
e. Identifies trends and opportunities for improvement. Responsible for identifying quality improvement opportunities and communicating these opportunities to the Manager of Quality and Safety and Director of Pharmacy if pertinent. Provides reports and recommendations to clinical staff, patient care areas, and hospital committees.
f. Leads, facilitates or participates root cause analysis surround a medication event. Reports findings to Director of Quality and Safety or other designee as assigned.
g. Identifies best practices for medication safety across the organization. Analyze current practices that contribute to medication errors and take proactive steps for prevention. Facilitate medication process quality improvement and assurance related to medication safety indicators of the National Quality form, the Agency for Healthcare Research and Quality, the Institute for Safe Medication Practices, the Institute for Healthcare Improvement, Leap-Frog, and the Joint Commission.
2) Regulatory Compliance
a. Co-leads, with the Systems Director of Pharmacy, compliance efforts with the Joint Commission Medication Management Chapter
i. Facilitates medication process quality improvement and assurance related to medication management indicators
ii. Participates in the SPHP (Joint Commission) regulation readiness activities such as tracers to identify medication management compliance opportunities.
iii. In conjunction with the Systems Director of Pharmacy, oversees the monitoring of compliance with proper medication control, safety, and security standards in pharmacy and patient care areas and serve as a resource. Facilitates changes needed to meet Joint Commission, New York Department of Public Health, New York State Board of Pharmacy, CMS and other government agency standards and laws.
3) Actively supports SPHP continuous quality improvement efforts designed to increase patient outcomes, increase patient satisfaction, and improve the utilization of SPHP human, capital, and physical resources.

b. Interviews, hires, trains, evaluates and when necessary, disciplines and discharges departmental personnel. Reviews all recommendations for discipline or discharge with the Systems Director of Pharmacy.
c. Monitors assigned staff for professional competency and compliance with regulations of outside agencies, e.g., OSHA, JC, NYSDOH, NYSSED (BOP), CMS, etc. and takes corrective actions as necessary.
d. Meets regularly with direct reports
i. Collaborates with other SPHP Managers & Supervisors

e. Develops action plans in response to employee engagement survey results.
2) Assists in resolving complex employee relations problems.
3) Responsible for providing a safe work environment in the departments by establishing, implementing, communicating, monitoring, and resolving employee safety issues and concerns.
4) Works in conjunction with the pharmacy management team to ensure all employees understand and contribute to the accomplishment of the department's goals and objectives, have performance plans linked to the department's goals and objectives, and receive at least an annual performance evaluation.
5) Responsible for developing and maintaining cooperative working relationships between Pharmacy department staff and other hospital personnel, physicians, and other influential contacts and ties to the department and its operations.
FINANCE
1) Budget
a. Works with Residency Program Director (RPD) for oversight of the SPH PGY1 Pharmacy Residency Program
b. Works with the Systems Director of Pharmacy for oversight of SPH Pharmaceutical spend
c. Assists the Director of Pharmacy in the review of SPHP productivity
d. Collaborates with the SPHP Pharmacy Management Team on assigned Trinity Health initiatives
GROWTH
1) Responsible for growth of Pharmacy services coordinated within SPHP.
SCHEDULE
The Clinical Pharmacy Manager is generally on-site weekdays. The manager is required to cover evening staff meetings and is encouraged to have regular communication (face-to-face and/or verbal communication) with staff. Occasional weekend/evening meetings/service is required.
Pay Range: $62.99 - $81.50
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
