The Pulmonary Hypertension and HHT Program at MGH cares for a subset of patients with pulmonary vascular disease characterized as PH, HHT, or CTEPH. The PH program is rapidly growing and MGH is on the forefront of offering all approved medical and surgical treatments for the disease, as well as being involved in numerous research trials to provide the most unique and up-to-date treatment options. The staff nurse is a registered nurse who is responsible for assuring competent, compassionate, individualized, nursing care for specific patients and their families in the ambulatory care setting. The nurse collaborates with practice leadership, members of the patient care team, and the patient to achieve optimal patient outcomes. Job duties include: * Triage patients and phone calls. * Coordinate the follow up of test results with provider guidance. * Handle prescription refills, injections, point of care testing. * Process medication refills including prior approvals. * Observes, reports, and documents relevant patient information in electronic medical records * Communicates the plan of care and other pertinent information to / from other patient care team members. * Provide patient and family teaching * Participates in and contributes to performance improvement activities. * Collaborates with practice leadership to identify his / her developmental level of practice and appropriate learning experiences to enhance development. * Contributes to relevant committees, professional meetings, continuing educational and / or formal academic programs. * Responsible for locating and liaising with community resources for patients and their families. * Participate in occasional cross coverage for General Pulmonary triage * Opportunities for some travel to educational conferences * Coordinate patient education. * Be available to inpatient units for any requested education. * Coordinate care with Vascular for patients that may need Balloon Pulmonary Angioplasty for CTEPH. * Coordinate care with thoracic and cardiac surgery for patients that may need Pulmonary Thromboendarterectomy. * Act as a point of contact for the HHT Center. * R.N., licensed in Massachusetts, with a minimum of five years of clinical experience * Bachelor's degree required; Master degree is preferred * Exceptional time management and organizational skills * Highly motivated, independent and capable of performing in a complex environment; Strong sense of initiative and perseverance to carry through to completion of tasks; Demonstrates forthrightness and integrity. Ability to identify issues/projects and initiate plans to address asking for direction when appropriate * Excellent interpersonal, written and oral communication skills; Good command of English language, including medical and scientific terminology. * Critical Thinking/Decision Making: Ability to appropriately evaluate all aspects of a situation and to independently make appropriate and timely decisions Working conditions: outpatient clinic with a shared office space. Schedule is four 10-hour shifts weekly, some hybrid work optional.
