Founded in 2015, we have expanded to approximately 100 employees and are continuing to grow. Our mission is to provide patients and their families with robust clinical services throughout their Acute Inpatient Rehab, Skilled Nursing, Long Term Care and Assisted and Independent Living stays. We coordinate with Post Acute Providers, Specialists, Surgeons, Physical, Occupational, and Speech Therapies, Case Management and Psychiatry to provide compassionate and coordinated care.DESCRIPTIONUnder supervision, the Licensed Practical Nurse acting as a Care Management Coordinator will perform a variety of medical and clinical support functions in the long term care/skilled nursing setting to provide support to our triage nursing team, providers, facility staff and administrative team.Job responsibilities include, but are not limited to:Facility liaison with excellent communication and relationship skillsComplete Chronic Care Management for LTC/SNF patients within assigned facilitiesProvide support to onsite providers including but not limited to:Collecting vital signsRelaying provider given verbal orders through EHR to facility nursing staffAssisting with entering medication ordersPerforming venipuncture and/or lab drawsIV insertion as licensure and certification allowsDocumentation of and assisting with provider-directed care planAttending IDT/care conference meetingsReviewing and documenting patient medical history in EHR systemAssisting in paperwork for discharge planning with facility staffProvide support to remote triage team with completion of a variety of tasks including but not limited to:Answering live triage line callsManaging incoming fax box and routing appropriatelyCompleting documentation in EHRManaging patient records for our practiceAssisting with any incoming medical records/authorization requestsParticipate in the Transitional Care Management program within assigned facilities- this includes but is not limited to:Assisting with the set up of home health services prior to dischargeMaking Home visits to patients as necessaryAssisting patient with provider directed DME orders for equipmentCoordinating care with outpatient specialtiesCoordinating a 7 day follow up visit via Telemedicine with Summit providersCompleting various 30 day follow up callsCoordinate any needed specialty visits for Summit's Long Term Care residents and facilitate with assigned facility for proper transportation.Taking care of other assigned tasks as needed.Accurate and daily reporting to supervisor.Job Qualifications:LPN active licenseMinimum of 1 year Case Management or Post acute care experienceExperience in use of Electronic Medical RecordsPreferred experience in ACO an/or value based care modelsStrong written and verbal communication skillsExperience with Google suites and smart sheets preferredBenefits:401(k)401(k) matchingDental insuranceHealth insurancePaid time offVision insurancePaid HolidaysLife insuranceHiring for multiple positionsPay rate $25-$32 per hour based on experience and licensure.Job Type: Full-timeSalary: $25.00 - $32.00 per hourBenefits:401(k) matchingSchedule:Monday to FridayWork Location: On the road
