Position Overview:

We are seeking a Care Navigator to join our expanding transdisciplinary team to support our patients and families affected by chronic conditions and serious illness related to aging. Centering on the needs and goals of the patient and the family caregiver, the Care Navigator helps guide our families through their unique journey by providing ongoing biopsychosocial support, personalized and culturally responsive care-planning, connection to local community resources, advocacy within social and health systems, coordination with primary care and disease stage specific education.

The Care Navigator will provide high quality care virtually via telehealth to patients/families living in home settings. You will work closely with our clinical team, that includes MD, Neuropsychologists, Nurse Practitioners and Social Workers to provide ongoing case management type support that will evolve with the ever-changing needs of our patients, and their caregivers.

Primary Responsibilities Include (but are not limited to):
  • Participate as an active member of our transdisciplinary team
  • Act as the primary point of contact for caregivers, families, support networks and those living with chronic conditions.
  • Actively supports caregivers, families and those living with chronic conditions to enhance quality of life and ensure safety in the home.
  • Conduct biopsychosocial assessments to identify strengths, goals, unmet health-related social needs, safety concerns, and to gain a comprehensive understanding of where our patients and families are in their journey.
  • Develop personalized, creative responsive care plans that align with our clients' values, cultural, emotional and spiritual needs.
  • Provide disease and stage-specific education, utilizing our extensive resource library.
  • Refer and connect family caregivers, and patients, to community- based services and supports.
  • Communicate with the patient's primary care physician and other healthcare specialists to ensure continuity of care for our clients and patients.
  • Provide caregiver education that focuses on identifying caregiver burnout, self-care, resiliency, skills training and support group services.
  • Track and maintain contact frequency based on program model and policy.
  • Participate in monthly transdisciplinary meetings to collaborate and coordinate care effectively
  • Provide accurate and efficient documentation for all interactions including the presenting issues, goal, intervention and action plan.
  • Follow established company policies, procedures and clinical protocols and uphold Magnolia's policy on client confidentiality.
Experience/Skills:
  • Preferably 2 years of experience working in a healthcare or community-based setting.
  • Ideally, 1 year of experience (professional or lived) in understanding the needs and challenges faced by family caregivers and older adults
  • Demonstrate a deep understanding of cultural humility and the ability to communicate and effectively interact with individuals from diverse cultures, abilities, genders, ethnicities, and races.
  • Experience working with Electronic Health Records (EHRs) a plus
  • Ability to handle interactions with caregivers and patients involving heightened emotions.
  • Strong general computer skills including the ability to navigate various different systems and resources, troubleshoot minor technology issues, and set-up own work equipment.
  • Attention to detail and effective time management and multi-tasking skills
  • Familiarity with healthcare industry, mental health, aging and disability services, or Alzheimer's disease
  • Proficiency in another language, other than English, is a plus but not required.
