Responsibilities:

Position Summary:
As the Care Coordinator, your primary responsibility is to lead wellness classes and coordinate the care of patients in assigned communities. You will work closely with the integrated care services team to ensure that residents receive holistic, individualized care. This role requires administrative, clinical, and educational expertise.

Essential Duties include the following:

  • Leads and organizes wellness classes to promote health and well-being among residents.
  • Assists in schedule management, ensuring seamless delivery of care and wellness programs.
  • Coordinates care by liaising between medical providers, nursing teams, and residents.
  • Obtains consent and all necessary intake paperwork from resident or POA prior to initiation of integrated services.
  • Performs data entry tasks and maintains patient electronic health record.
  • Documents residents' medical information, including medical history and health insurance information.
  • Manages all internal and external communications concerning resident health and wellness.
  • Collaborates with the Nurse Practitioner and communicates regularly with the executive director and/or director of nursing regarding any areas of concern or successes.
  • Maintains strong relationships with physicians, care staff, administrative teams, and resident family members.
  • Records patients vital signs, documenting physicians notes and changes in medical conditions.
  • Complies with HIPAA and PHI guidelines, ensuring patient/resident confidentiality at all times.
  • Performs other related clinical, administrative, and educational duties as assigned.

Qualifications:

  • High school diploma or equivalent; completion of a Medical Assistant program preferred.
  • Certification as a Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred.
  • Previous experience working in a geriatric setting or with elderly residents highly desirable.
  • Exceptional communication and interpersonal skills are required along with the heart and passion to work with older adults while providing the best care possible.
  • Must have the ability to remain calm in stressful situations, be flexible, work well with many interruptions, and have exceptional multi-tasking skills
  • Must exhibit and promote a high level of professionalism, customer service, curiosity, and friendliness in all interactions with employees, residents, and visitors throughout the organization
  • Strong attention to detail and the ability to multitask in a fast-paced environment
  • Respect for the principles of patient/resident rights and confidentiality, including adherence to HIPAA regulations
  • Flexibility to adapt to changing priorities and willingness to work as part of a collaborative team.
  • CPR certification may be required
  • Proficiency in Internet browsers (e.g. Explorer, Chrome, Fire Fox), as well as advanced knowledge of Microsoft Office programs: Outlook, Excel, Word and Publisher applications.
  • Experience in working with various EMR and medical billing systems.
