Plusses:

Extensive knowledge of principles and practices of a Registered Nurse, Program guidelines, and the Office of Mental Health regulations. Ability to provide psychiatric and physical health need assessments making appropriate referrals for those with severe and persistent mental illness and/or histories of substance abuse and trauma. Thorough knowledge of medication management and administration. Responsible, organized, effective and efficient. Ability to perform within a team and accomplish multiple assignments simultaneously. Skill to communicate effectively in writing, vocally, and to listen actively. Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are diagnosed with severe and persistent mental illness. Ability to deal tactfully and diplomatically with other Comunilife employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.

Day-to-Day Responsibilities:

1) Responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and supportive services. 2) Provides comprehensive primary health care, including but not limited to, case finding, health screening, health promotion, physical and psychosocial assessment, case management, and overall management of medications. 3) Ensure and maintain principles, and procedures that are in agreement with a Nursing licensure, Program guidelines, and the Department of Mental Health and Office of Mental Health regulations. 4) Develop, maintain, and revise written plans of care and general health care records. 5) Engage new clients in the continuum of care through outreach. 6) Collaborate with other health care providers and make appropriate referrals. 7) Serve as client advocate specifically in accessing the health care system. 8) Provide communication to team members and participate in community and apartment visits to clients. 9) Coordinate services with other community mental health services such as, inpatient, outpatient, emergency, or day treatment. Assist in the development of a program that closes gaps in the continuum of services. 10) Affirm that all record keeping activities, including but not limited to, databases, daily logs, progress notes, and client charts are appropriately maintained and current. 11) Assist in the training of team members in the range of health promotion and illness prevention, treatment, rehabilitation, and support services available. 12) Serve as a health care resource to clients and extended families. In both self and group processes. 13) Assist in the preparation of any and all clinical correspondence relative to the activities of the Team as requested by the City, State, or Federal funding sources. 14) Assist the Program Team with special projects.
