As a Care Navigator, you'll play a critical role in guiding patients through their healthcare journey. You'll be the main point of contact for patients, assisting with everything from appointment coordination to understanding treatment plans. Working closely with healthcare providers, you'll advocate for patients, provide education, and connect them to essential resources, helping to remove barriers to care. Your Impact

    • Serve as the primary point of contact for patients, caregivers, and providers, helping facilitate seamless transitions of care and addressing any barriers along the way.
    • Conduct initial assessments to understand patient needs, goals, and any obstacles they may face in accessing care.
    • Develop personalized care plans in collaboration with healthcare teams, tailoring support to meet each patient's unique needs.
    • Coordinate appointments, referrals, and tests to ensure patients have timely access to the care they require.
    • Advocate for patients to make sure they receive necessary treatments, medications, and resources to support their health journey.
    • Provide education to patients and caregivers on health conditions, treatment plans, and effective self-management practices.
    • Track patient progress and outcomes, adjusting care plans as needed to promote the best possible health results.
    • Accurately document patient interactions, care plans, and interventions in electronic health record (EHR) systems.
    • Work with insurance providers and connect patients to community resources to address social determinants of health and ensure access to necessary supportive services.
    • Participate in care conferences and team meetings to discuss patient goals, progress, and discharge planning.
    • Perform other duties as assigned by management needed to help drive to our Vision, fulfill our Mission, and abide by our Organization's Values.
Your Credentials
    • Passion for healthcare and a strong desire to support patients in navigating their care journey.
    • Bachelor's degree in a healthcare-related field preferred (e.g., Public Health, Health Administration, Social Work) or equivalent experience in a healthcare setting.
    • Excellent interpersonal and communication skills, with the ability to build rapport with patients, caregivers, and healthcare providers.
    • Strong organizational skills and attention to detail, with the ability to manage multiple tasks and coordinate appointments and services efficiently.
    • Basic knowledge of healthcare terminology and an understanding of healthcare workflows.
    • Familiarity with electronic health records (EHR) systems is a plus, with the ability to learn new software quickly.
    • Ability to work effectively in a team-oriented environment, collaborating with multidisciplinary teams to support patient care.
    • Problem-solving skills and the ability to advocate for patient needs.
    • Flexibility and adaptability to handle a dynamic healthcare environment.
    • Experience with insurance coordination and knowledge of social determinants of health is a plus, but not required for entry-level applicants.
