Description: Responsible for the nutritional management of patients at Brigham and Women’s Crohn’s and Colitis Center. Provides direct patient care to patients through nutritional evaluation and follow up education with patients focused on symptom management, healthy eating and nutritional deficiencies. Performs activities in the setting of a multidisciplinary care team and will be expected to work closely with physicians, advanced practice practitioners and nurses in the Crohn’s and Colitis Center. This position will require knowledge of gastrointestinal and inflammatory bowel diseases and their relevant dietary recommendations / protocols. The successful applicant must possess strong interpersonal and teaching skills to communicate information to patients and colleagues and will participate in program development and nutrition related initiatives. Evaluation and formulation of individual dietary treatment plan(s) for patients seeking care at the Crohn’s & Colitis Center and long-term co-management with the rest of the center’s team, focused on reducing inflammation, preventing malnutrition, healthful weight gain and managing GI symptoms with diet. Provides education of and guidance to follow IBD specific diets for interested patients. 2. Recommends appropriate nutrition support based on subjective and objective data. Documents, monitors, and communicates nutrition care plans through written and verbal systems. Documents in medical record based on established standards of practice and within established time frame based on standards of care. Executes nutrition interventions in accordance with the latest research in nutrition support, physiology and scientific knowledge. Provides patient support, motivation and education aimed at improving and enhancing health and gastrointestinal outcomes, in both individual and potentially group settings. Evaluates appropriateness and effectiveness of therapy for patients receiving home nutrition support. Adjusts the nutrition prescription regimen as needed to meet patient’s nutritional requirements and re-educates patient/caregiver as indicated. Follows progress of patients during follow-up visits and makes appropriate changes in care plan as appropriate. Communicates with multidisciplinary staff regarding any nutrition- or health-related issues influencing the overall progress of the patient. Participates in patient care meetings to discuss patient care issues and in weekly meetings with the multidisciplinary psychosocial and nutrition program to review enrolled patients. Participates in IBD center program events. Prepares patient education materials to distribute to patients. Follows the standardized clinical template provided by the Crohn’s and Colitis Center. Performs other duties as assigned Qualifications: Baccalaureate Degree Registered and Licensed Dietitian A minimum of 1 year experience in counseling and behavior modification techniques.
