The Director of Rehabilitation will seek to return the resident to the optimal physical function and facilitate achievement of resident, Physician, and therapy goals. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Rehabilitation Policies and Procedures.Qualifications/ Responsibilities:Must possess hold a valid and current license as a therapist or therapist assistant granted from the state of Ohio.Participate in community planning related to the interests of the facility and the services and needs of the resident and familyParticipate in discharge planning, development and implementation of social care plans and resident assessmentsInterview resident/families to obtain social historyRefer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the residentProvide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the residentProvide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programsPrevious experience required, experience in PDPM preferredShift & Wage:Full-Time Opportunity AvailableShifts Needed: Day ShiftCompetitive SalariesAbout Us:We strive to be the leader in the long-term care industry operating in 6 different states.
