Medical Social Worker, Mercer County

Medical Social Worker needed for full-time, temporary position in Mercer County.
9am-5:30pm shift. A weekend rotation is required (approximately every 4th weekend).
Medical social workers must have acute care hospital experience. Develops a plan appropriate for resident discharge. Ensures all community services and equipment needs are arranged before discharge. Support and assist the team in care needs and understand the discharge plan. Completes all necessary paperwork for the patient and family concerns.
Position Requirements
NJ LSW or NJ LCSW with medical acute inpatient experience. Behavioral health experience is a plus. Excellent oral and written communication skills. Knowledge of community resources. Experience working as part of an interdisciplinary team.
