Position Summary: The Pharmacist - Inpatient role helps provide high quality pharmaceutical services that foster efficacious, safe and cost effective drug use for our patients. Plays an integral role in the coordination and delivery of clinical and distributive services. Primary duties include performing clinical review and order verification, following pharmacy clinical protocols/policies, completing interdisciplinary round attendance, providing drug information for providers and nurses, overseeing central pharmacy activities, and providing guidance and oversight for pharmacy technicians. Actively participates in medication safety and quality initiatives. Maintains optimal pharmacy practice through continued growth and development and completes all required CEs for licensure in the state of Montana and those required by BHDH Primary Stroke Certification. Works closely with Clinical Pharmacy Supervisor and other members of the pharmacy leadership team.Minimum Qualifications:Required+ Bachelor of Science or Doctorate of Pharmacy+ Montana Licensure (Pharmacist) or eligible to obtain licensure upon hire+ American Heart Association BLS required within one year of hire+ American Heart Association ACLS required within one year of hirePreferred+ PGY1 residency or equivalent experienceEssential Job Functions:In addition to the essential functions of the job listed below, employees must have on-time completion of all required education as assigned per DNV requirements, Bozeman Health policy, and other registry requirements.+ Demonstrates competent knowledge and skills of EPIC, electronic health record, required pharmacy software, and automated-dispensing systems. Responds to in-hospital alerts including but not limited to code blue, code blue PALS, stroke alerts, RRTs and traumas. Processes medication orders and monitors workflow activity to maintain timely medication services. Remains competent in sterile room processes, IV admixture and hazardous medication handling and preparation. Maintains bi-annual PATT/GFS requirements. Demonstrates competency of after-hours procedures for outpatient prescription filling.+ Provides drug information to providers, nurses and other ancillary personnel. Promotes strong interdisciplinary communication and attends interdisciplinary rounds. Actively contributes to medication safety and quality initiatives. Reports adverse events and good catches. Identifies areas of improvement and updates for patient care based on national guidelines. Contributes and participates in pharmacy education among peers and for residents and students. Encouraged to participate in state and/or national pharmacy associations.+ Maintains optimal pharmacy practice through planned and continuous growth and development. Seeks new learning experiences. Meets C.E. requirement. Contributes and participates in pharmacy education.+ Other duties as assigned.+ Essential Job Functions – Transition of Care Pharmacist:+ The transitions of care (TOC) pharmacist works closely with an interdisciplinary team of physicians, case managers and nurses to ensure the safe transition of inpatients at discharge. The TOC pharmacist role involves reconciling admission and discharge medications, evaluating appropriateness of discharge therapies, providing medication counseling, assisting patients in obtaining the necessary medications at discharge, and coordinating with receiving facilities to ensure prescription needs are met.+ Demonstrates competent knowledge and skills of EPIC, electronic health record, required pharmacy software, and automated-dispensing systems. Keeps up to date on EPIC upgrade knowledge and relays questions and concerns to pharmacy super-trainer group. Performs clinical review and verification of provider orders.
