Work in close collaboration with physicians, nurses, and other healthcare professionals. DUTIES & RESPONSIBILITIES:1. Focus on patients recently discharged who are at high risk for readmissions.2. Take a leadership role in preventing medication-related problems as patients transition through health care settings and home, and make sure the medical management of the patient remains cohesive throughout changes in their care setting.3. Coordinate/participate in activities related to improving outcomes for targeted disease management initiatives leading to improved health (such as COPD, Asthma, CHF, and Diabetes).4. Provide cost-effective pharmaceutical care to patients receiving drugs that require clinical pharmacokinetic/anticoagulation monitoring.5. Engages in active patient-centered medication therapy management through transitional care including medication reconciliation services and discharge assistance: patient counseling on appropriate medical therapy, medication history, admissions and discharge medication reconciliations, facilitation of patient care, coordination of benefits, authorizations and formulary management.6. Coordinate/participate in activities related to improving outcomes for targeted disease management initiatives leading to improved health (such as COPD, Asthma, CHF, and Diabetes).7. Ensure medication errors and ADRs are reported and pathways developed to prevent future occurrences.8. Support of quality improvement projects and research initiatives.9. Performance of medication use reviews to assess cost effectiveness and medication appropriateness.10. Coordinates medication reconciliation across the continuum of care and monitoring to improve clinical outcomes and to help prevent unnecessary readmissions, lapses in care and adverse drug reactions.11. Provide cost-effective pharmaceutical care to patients receiving drugs that require clinical pharmacokinetic/anticoagulation monitoring.12. Optimize cost savings and cost avoidance via formulary adherence and best practice clinical guidelines.13. Develop and conduct target drug programs and medication use evaluations as needed and report results to the Pharmacy and Therapeutics Committee.14. Develop clinical guidelines and protocols for the institution.15. Monitor the use of expensive medications to assure there is use consistent with approved criteria.16. Provide highly specialized education and training support to other professional services in the broad area of advanced pharmacotherapy.17. Precept Pharm.D. Clerkship students on the Internal Medicine Service.18. Provide education to staff pharmacists as needed.19. Maintains responsibility for both the clinical and the distributive activities of the Pharmacy Department as necessary.20. Assists with staffing as deemed necessary.21. Perform other related duties as required.Minimum QualificationsFor Level I: 1. Possession of a valid NY State license to practice pharmacy. For Level II: In addition to meeting the qualification requirements of the lower level: 1. Two years of post licensure experience; or 2. Completion of a post graduate specialty residency or fellowship program; or 3. Current Pharmacy Board Certification. For Level III: In addition to meeting the qualification requirements of the lower levels, an additional: 1.
