Job Title: Lead Case ManagerFLSA Status: Full Time - non-exemptReports to: Program SupervisorSchedule: FT, VariesSupervises: Case ManagerRate of Pay: $29.35Closing Date: 2/15/2024Benefits: Standard; Full-Time, Non-Exempt employees are eligible for butnot limited to the following:Health, vision, dental, life as well as voluntary life and disabilityinsuranceSick leave benefit - 1 day per month, 12 sick days per year (accrual andavailability begins at hire)Vacation benefit - 10 vacation days per year, accrued at the rate of .0385hours for each hour worked, excluding overtime (accrual begins at hire butmay not be used until the completion of six months of employment)One floating day off for use (accrued immediately, and again annually,but may not be used until the completion of the initial three-monthintroductory period of employment)Pension Plan (after one year of continuous service)Voluntary Tax-Deferred Annuity Plan (403(b)plan)Function: This is a full time, non-exempt, hourly position, 40 hoursa week with vacation and sick time as well as medical, dental, andoptional insurance coverages.The Lead Case manager conducts intake and ongoing assessments, prepares caseplans while assuming a leadership role and directly oversees programing casemanagement staff and interns. Provides individualized and person-centered casemanagement services with the goal of moving families toward self-sufficiencyand stability as quickly as possible. This position is critical in modelingand providing guidance and support to staff in a shelter utilizing the HousingFirst approach to homelessness assistance. This full time, non-exempt rolereports to the Lambuth Family Center (LFC) Program Director.Duties and Responsibilities:Lead Case ManagerFacilitate weekly case coordination meetings and other programming meetingswith case managers, education & activity coordinator, interns and otherrelevant staff (in conjunction with the LFC Program Supervisor and Director)Carry a caseload and create individualized, client centered case managementservices and case plans.Conduct one time and ongoing assessments including Health Hope Index, SelfSufficiency Matrix and VI-SPDAT.Create community connections for referrals and partnerships. Serve as aliaison for residents.Manage case files, creating, preparing, and maintaining accuracy;provide monitoring and oversight for all case management files, ensuringaccuracy and providing guidance when needed. Makes recommendations forimprovement of file/data storage and systems.Assist in the coordination of Continuum of Care for program participants as itrelates to community partner agencies and MDHI.Provide oversight to case management from intake to program exit; providerecommendations and strategies to improve and streamline systems and ensureall components are client centered.Program Oversight:Acts as direct supervisor for programming staff including case managers, andinterns.Responsible for approving and submitting time sheets and time-off requests forcase management staff.Provide support, encouragement, and crisis intervention to staff andresidents as needed. Model non-violent crisis intervention strategies.Train and onboard new programming staff in the areas of Housing First,Trauma Informed Care, Crisis Management, Data management systems, etc.Consults with the LFC Program Director on program related decisions.Work with LFC staff to prepare grant and contract reports in a timely manner.Other duties as assigned.
Qualification and Education Requirements:Minimum of 3 years case management experience, preferably with homelesspopulations. Minimum 1 year experience in a supervisory role. Bachelor'sDegree in Social Work or Human Services related field required. Master's inSocial Work or similar degree, preferred.Qualifications
