Description

The Patient Navigator supports patients and their families with educational resources about their diseases and helps to navigate the many paths of a healthcare diagnostic and treatment process.

Essential Duties and Responsibilitiesinclude the following:
  • Identifies support groups appropriate for patients and families and coordinates scheduling.
  • Removes obstacles to treatment by scheduling appointments, coordinating referrals, translation, and other needed services as necessary.
  • Tracks and documents metrics and outcomes.
  • Call patients to schedule an office visit or remind them to pick up their medication.
  • Order and schedule various procedures, tests and screenings.
  • Identify and track progress on important gaps in patient care.
  • Maintain a record of patient interactions and communicate with providers using electronic health records.
  • Educate patients on their healthcare options, insurance benefits, and common medical conditions.
Required Qualifications:
  • Personal strength in empathy and forming relationships.
  • Effective skills in persuasion and encouragement.
  • Knowledge of basic principles of health promotion, prevention and motivation.
  • Knowledge of protocols in general and an ability to quickly gain specific knowledge of the Center's current operational protocols.
  • Knowledge of medical terminology and appropriate abbreviations.
  • Ability to read and comprehend medical instructions, short correspondence, and memos.
  • Ability to effectively present information in small group situations to patients, Center staff, and the general public.
  • Proficiency in managing electronic medical records.
  • Must have proven skills in working independently, self-motivated and goal oriented.
  • Ability to work flexible hours, as responsibilities may dictate.
  • High degree of accuracy, efficiency, organization, thoroughness and attention to detail required.
  • Ability to work independently and as part of a team.
Education/Experience Requirements:

Bilingual - English and Spanish.

High School diploma or general education degree (GED).

Minimum of two (2) years equivalent work experience in the healthcare industry.

* American Cancer Society Leadership in Oncology Navigation (ACS LION) certification must be obtained within one (1) year of employment.*
