Delegating Nurse
1. Performs initial and 45 days assessments on-site and at a minimum of every forty-five (45) days
2. Assesses the competency of certified medicine aides and certified medicine techs every 45 days.
3. Assesses the tasks delegated to determine if it is still reasonable and prudent to delegate every 45 days.
4. Assesses residents and medications on admit or readmission within 14 days according to state regulations.
5. Assesses residents and medications on all self-medicating residents every 90 days.
6. Performs all assessments in an accurate and timely manner
7. Participates in the on-site 6 month review with the ALM and as needed as concerns arise.
8. Recommends changes in level of care within the facility to meet the needs of our residents as needed.
9. Manages the clinical oversight of resident care and consults with residents, families, physicians and pharmacy personnel as needed. This includes safe storage of medications
10. Implements resident's plan of care as well as related daily/weekly schedule based on lifestyle preference needs and physical assessment and attends Service Plan meetings.
11. Abide by the Neighborhood Approach to caring for people living with a memory challenge or diagnosis, to include:
a. Being present during all meal times to assist with dining services, bring residents to eating areas or delivering to resident's rooms.
b. Ensuring that special diets are served in accordance with the prescribed diet, including fluid, responsible for meal accuracy prior to delivery.
c. Monitor all likes and dislikes and problems with chewing/swallowing etc.
d. Communicate pertinent information daily to AM/PM Host/Hostess, supervisor and/or dietician.
e. Assist with serving meals and beverages collaboratively with the Host/Hostess or as needed based on resident request.
f. Ensure proper server and food handling.

12. Conducts and documents a thorough assessment of each resident's medical status upon admission and throughout the resident's course of treatment.
13. Provides safe, direct physical care of the resident.
14. Maintains a clean, safe and orderly resident environment by keeping the area clean and disposing of soiled, contaminated items.
15. Reports malfunctioning, broken, or hazardous equipment or conditions to the supervisor.
16. Observes policies and procedures as demonstrated through performance of procedure and tasks.
17. Completes required documentation in an accurate and timely manner.
18. Oversees care delivery for the shift and receives/shares pertinent information with the prior and oncoming shift.
19. Other duties as assigned.
