Palatka Center for Rehabilitation and Healing is now hiring an experienced MDS Care Plan Coordinator who is committed to providing the best life and care for our residents. Under the: direction of the Director of Nursing, the MDS Care Plan Coordinator is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning. The MDS Coordinator can be a LPN or RN

MDS CARE PLAN COORDINATOR QUALIFICATIONS:
  • Must possess current and valid state Registered Nurse ( RN ) license OR Active Licensed Practical Nurse ( LPN ) License by the State
  • MDS Coordinator experience is REQUIRED
  • Must be able to relate positively and favorably to residents, families and outside agencies and to work cooperatively with other associates
