We specialize in helping patients heal their wounds by providing the best possible treatment options in the comfort of their own home. This is an excellent opportunity for a Physician's Assistant or a Nurse Practitioner to join a growing organization whose mission is to change how wound care treatment is delivered and increase access to high quality wound care. This position a is full-time salaried position with the flexibility to work Monday-Friday. No on-call No mandatory weekends, evenings, or holidays Daily local travel is required to patient's homes. All mileage is reimbursed. Equipment, medical supplies, and training will be provided. *Qualifications* * Graduate of an accredited nurse practitioner or physician assistant program * A clinical background in adult, family, geriatric, podiatry, or vascular specialties * At least one year as an advanced practice provider * Board Certified/Prepared through AANP, ANCC or NCCPA * Wound care experience a plus * Nurse Practitioner national certification as ANP, FNP, or GNP required or ability to obtain * Current, unrestricted license in the state of assignment or the ability to obtain license * Ability to obtain and/or maintain State DEA licensure / Prescriptive Authority post - where applicable *Practice Details* * The PA or NP is an experienced member of the health care team who provides advanced wound care to patients in their home setting. * Assesses health status for wound care patient populations using advanced practice knowledge and skills. * Develops and implements an individualized patient plan of care including coordination of care between visits. * Thorough Assessment: Conduct comprehensive patient assessments, paying special attention to factors that increase the risk of wound formation. * Advanced treatments such as surgical debridement or CAMP. * Medication Management: Order necessary medications and provide clear instructions for their use. * Educate and Communicate: Educate nursing staff, and maintain open communication with primary care physicians, families, and clinical teams as required. * Contribute to effective and efficient practice management policies and practices that support patient access to medical care. * Proven ability to function at highest level within scope of practice. * Patient education and pro-active disease management experience. * Demonstrates ability to provide care or service adjusting approaches to reflect developmental level of population served. * This individual must show commitment to exemplary patient care. *Benefits & Perks* * Medical insurance * Dental insurance * Vision insurance * Flexible Spending Accounts * Short and Long-term disability * Legal Protection Plan * Supplemental Insurance Plans * Paid Time Off (PTO) * Holidays * Flexible schedule * Day Shift no evenings or weekends * Mileage reimbursement To learn more about Mendota Health, please visit our website: www.mendotahealth.com *Job Type:* Full-time *Pay:* $130,500.00 - $200,000.00 per year *Medical Specialty:* * Wound Care *Experience:* * Wound care: 1 year (Required) * NP or PA: 1 year (Required) *Ability to Commute:* * Hartford, CT *Willingness to travel:* * 75% (Required) *Work Location:* On the road Job Type: Full-time Pay: $130,500.00 - $200,000.00 per year Benefits: * 401(k) * 401(k) matching * Dental insurance * Employee assistance program * Flexible schedule * Flexible spending account * Health insurance * Life insurance * Paid time off * Retirement plan * Vision insuranceApplication Question(s): * Are you licensed as a PA or NP in the state of CT? * Do you have any wound care experience?
