Join us in our mission to make a difference in the lives of our patients and their loved ones.Overview:The Nurse Practitioner is a registered professional nurse with advanced graduate education and clinical training who provides comprehensive hospice services under the supervision of a collaborating physician. In this role, you will evaluate and assess health history, promote health, diagnose medical conditions, manage health problems, prescribe medications or treatments, and coordinate care with patients, families, and other healthcare practitioners. You will also visit patients, perform physical assessments, and determine hospice eligibility in consultation with the interdisciplinary team.ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:Patient Care:Provide medical care, palliation, and management of terminal illness in compliance with the state Nurse Practice ActConduct history and physical examinations, interpret test results, diagnose health conditions, and document findingsDevelop individualized plans of care, prescribe medications or treatments, coordinate services, and ensure symptom controlComplete face-to-face visits to assess hospice eligibility and provide clinical information for recertificationCollaborate with the interdisciplinary team to review and revise the patient's plan of careCommunicate and coordinate with attending physicians, healthcare practitioners, and community resourcesEducate patients, families, and caregivers on preventive care, medical and psychological problems, and spiritual needsAdditional Duties:Maintain knowledge of and compliance with current Medicare/Medicaid and state/federal regulationsEnsure compliance with Medicare conditions of participation and other healthcare regulationsComply with HIPAA requirements and maintain accurate documentationParticipate in quality improvement initiatives and professional development activitiesPOSITION QUALIFICATIONS:Graduation from an accredited School of Nursing and accredited Nurse Practitioner ProgramCurrent nursing licensure in State and CPR certificationMaster's degree with a minimum of one (1) year Nurse Practitioner experience and hospice or palliative care experienceAccreditation from an approved certifying body for advanced practice nursingCertification in a specialist area preferred (e.g., Hospice and Palliative Nursing, Pediatrics, or Geriatrics)Excellent observation, communication, problem-solving, and nursing skillsAbility to lift, position, and transfer patients, as well as perform required nursing skillsValid driver's license and insured automobile in good working orderIf you are a motivated and dynamic Nurse Practitioner looking to make a difference in the lives of hospice patients, we want to hear from you.
