ITCD Specialist

The ITCD Specialist will be responsible for providing specialized services for the treatment of co-occurring substance use and mental illness to individuals enrolled in the Community Psychiatric Rehabilitation Program.

This is a full-time, benefit eligible position working Monday - Friday.

Responsibilities Include:

Service provision based on multidisciplinary rehabilitation plan to include, but not limited to, assessing level/stage of treatment, administering assessment tools, providing individual co-occurring counseling, arranging and providing co-occurring educational and process counseling groups, preparing and/or participating in development of individualized treatment plans and treatment reviews
Providing outreach and engagement services as needed
Providing CPRC community support services as indicated in the consumers treatment plan
Assist in the assessment and documentation of consumer eligibility for services
Responsible for appropriate documentation and recording within the case record according to policy
Participate in ongoing clinical supervision
Participate in weekly team meetings
Assist client with transportation as clinically indicated; Driving is an essential function of this position
