An exempt, management position responsible for leading and managing the quality services provided with physician practice setting to obtain the best possible outcomes.Quality and Care Coordination Manager Responsibilities and Job Duties Under the guidance of the Department Director, this position is responsible for oversight of daily operations, staff productivity, manage the Medicare Annual Wellness schedule. Ensure Medicare Annual Wellness receptionist is fully trained to assist with ACO portal updates and assist with Diabetic Education incoming referrals. Will set short term goals and deadlines, organize day to day operations, monitor employee performance/productivity, provide feedback and coaching, complete performance management activities for direct report (e.g. evaluations). Directly manages the Quality Staff that oversee Value Based Contracts, directly manages the Clinical Services Specialist. Will work in conjunction with Population Health Nurses to ensure all Care Coordination programs are being documented accurately meet quality documentation requirements. Ensure Medicare Annual Wellness receptionist is fully trained to assist with ACO portal updates and assist with Diabetic Education incoming referrals. Will assist to develop practice based clinical protocols to ensure consistent delivery of quality services. Coordinates with Medical Staff and providers to ensure quality patient care and services are being provided. Perform and/or have oversight of quality reporting, analyzing, planning, and educating. Support management functions, attend leadership meetings, management meetings and assist in yearly performance evaluations for staff.Quality Data Management Serves as subject matter expert on quality requirements and measures. Manage/Coordinate/participates in the coordination of quality improvement activities and actively seeks educational resources that enhance and maintain current knowledge and skills. Under the guidance of the Department Director, this position is responsible for oversight of daily operations, staff productivity, manage the Medicare Annual Wellness schedule. Manage and Provide quality and care coordination education/training for teams across the continuum of care. Provide EPIC education that pertains to quality measures. Ensure Medicare Annual Wellness receptionist is fully trained to assist with ACO portal updates and assist with Diabetic Education incoming referrals.Manage Contracts Manage Medicare Advantage/Medicaid/Commercial Quality contracts. Monitor/assign employees to quality programs. Collaborate with credentialing to ensure we maintain up to date provider rosters. Overall Manage/Facilitate activities and meetings as needed to ensure rapid forward movement of sustainable solutions. Identifies streamlining and process improvement opportunities. Support Quality Director and clinical staff as appropriate.QualificationsQualificationsMinimum of 5 years in healthcare experienceMinimum of 1 year experience in Quality and Care Coordination Current CMA certificationExperience in organizing and prioritizing quality managed care (pay for performance) contracts and processes.Proficient and knowledgeable with Medicare annual wellness visits, eligibility verifications, ability to educate staff/patients on importance of preventive care.Must possess knowledge and understanding of ACO metrics and guidelines.Expertise in EPIC charting and knowledge of charting processes that meet MIPS dashboard measures.
