We are currently searching for an experiencedNurse Practitioners or Physician Assistants to engage in Medicare Wellness Visits for part-time hours for an 11-month contract at our contracted partner clinics. **Target start date is January 2025** **This is a part-time position of 3 days per week, 8-hour days (24 hours per week)** **Must cover the following 3 days a week: Monday, Tuesday and Friday** **This role will cover our partner clinic for Medicare Annual Wellness Visits** Responsibilities include:Perform Wellness Visits in partner primary care clinic and facilities in the Montecito Point community. Drive to designated facility locations in your assigned geographic area.Review patient medical and family history.Review of current providers and prescriptions.Perform height, weight, blood pressure, and other routine measurements.Deliver personalized health advice.Review risk factors and potential treatment options.Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.Provide referrals to appropriate medical resources. Utilize our EMR system to document patient visit/interaction accurately and timely.Knowledge of HCC coding.Other duties as assigned. Qualifications and experience:Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.Current, unrestricted NP or PA license to practice in California, including DEA license.Minimum of 2 years of experience in primary care, hospice, or urgent care/outpatient setting.Minimum of 1 year of previous experience utilizing an EMR system.Fluent in Spanish preferred.Good working knowledge of HCC coding.Strong computer skills including MS Office and Outlook is strongly desired.Great verbal and written communication skillsCurrent drivers' license, auto insurance and reliable transportationCompassionate and caring attitude with a desire to live our mission, vision and values.Tuberculosis Screening PolicyTo ensure the health and safety of our members, if you are selected for this position, your job offer with Homebase Medical will be contingent on providing proof of Tuberculosis screening upon hire or providing proof of a negative screening within the last year.
