A pharmacist with us can either be involved directly in services themself or by freeing others. Services our pharmacy is involved in including proactive supplement education, micronutrient testing, hearing aid screening and fitting, health coaching and monthly group discussions, Medicare D plan consultation, MTM, CMR and WPQC services. Summary: In a retail setting, the staff pharmacist administers prescribed medications. Additionally accountable for the transfer of prescriptions, the management of incoming physician communications, the recording of data into the computer, and the execution of Drug Utilization Review. Must possess a bachelor's degree and/or an advanced degree in pharmacy and be licensed to practice in Wisconsin. Familiar with a diverse array of the field's concepts, practices, and procedures. Utilizes a wealth of experience and sound judgment to establish and achieve objectives. Executes an assortment of obligations. Directs and supervises the activities of others. A broad spectrum of creativity and flexibility is anticipated. Typically, this individual is responsible for reporting to a manager or the chief of a unit or department. Essential Duties of the Position: Conducts an assessment of physician medication orders to determine the appropriateness of the substance, dosage, potential interactions, route of administration, and stability. Provides the physician and facility personnel with recommendations for therapy modifications as necessary. Conducts Drug Utilization Review (DUR) on all prescriptions and reviews clinical revisions to ensure that there are no adverse drug interactions, utilizing patient medical information. Maintains a patient/resident database that includes pertinent clinical and laboratory data, current and past medical history, and concurrent medication (including over-the-counter and home remedies). Collaborates with the appropriate pharmacy technicians to reconcile the Controlled Substances Log(s) with the inventory on hand and the prescriptions dispensed in order to ensure compliance with state and federal regulations. Collaborates with the Chief RPh to guarantee that all dispensing and procurement activities adhere to cost containment strategies, including Cycle-Fill, Front-End Process, and Pre-Pack. By responding to drug-related inquiries and concerns initiated by internal (Technicians, Nursing Staff) and external (Directors of Nursing, Administrators) customers, the individual provides clinical expertise. Provides clinical and technical support in conjunction with the healthcare team as required. In the medical record, all communication with physicians, nurses, other members of the healthcare team, patients/residents, and their family members is documented. Obtains and maintains all necessary documentation in compliance with the policies of MD Group LLC and federal and state regulatory agencies, as needed. Ensures the safe procurement, storage, distribution, and use of drugs and biologicals, as well as the disposal of drugs, through the implementation of policies and procedures. Assists facility/agency staff personnel in the implementation of secure and effective procedures for the procurement, maintenance, administration, and disposal of drugs.
