Overview of Responsibilities:
  1. Provide regular and dependable attendance.
  2. Transport and transfer patients.
  3. Disinfect equipment.
  4. Prepare and straighten treatment areas
  5. Evaluate each patient utilizing applicable diagnostic, prognostic, and functional ability tests.
  6. Re-evaluate patients status at appropriate intervals.
  7. Interpret evaluation data, history, and applicable medical documentation to determine rehabilitation potential and potential benefit from intervention.
  8. Identify physical, social, and psychological needs of the patient, family, and significant other.
  9. Utilize patient/family caregiver input in setting functional rehabilitation goals.
  10. Document goals in functional, measurable terms with stated time frames.
  11. Incorporate information gained in evaluation and assessment to determine appropriate, specific treatment plan for intervention.
  12. Involve patient/family caregiver in treatment planning.
  13. Demonstrate competence in performing all procedures and treatments required in area of practice including but not exclusive to those listed in the Competency Inventory.
  14. Demonstrate awareness of continuity of patient care by following established plans.
  15. Modify patient care based on continual assessment of the patient and effect of treatment intervention.
  16. Demonstrate skills in identifying, interpreting, and incorporating patients psychological, physical, and social responses to intervention.
  17. Provide and/or recommend equipment or supplies for home use.
  18. Provide education appropriate to each patients level of understanding regarding evaluation, treatment, and self-responsibility.
  19. Instruct in home treatment program to facilitate rehabilitation and promote independence in care as appropriate.
  20. Collaborate with patient, caregivers, and other health care professionals in planning discharge from rehabilitation services.
  21. Initiate discharge planning at time of initial evaluation.
  22. Confer and communicate with referring physicians and other appropriate persons through the use of progress notes, letters, evaluations, team meetings, and direct contact concerning the proposed course, progress, and discharge plan of each patient.
  23. Complete clear, concise, and clinically useful documentation in medical record in a timely manner according to department procedures.
  24. Provide clinical supervision to O.T.A.s regarding patient assessment, implementation of treatment plan and protocol, and decision making.
  25. Counter-sign O.T.A. documentation in a timely manner in accordance with department procedure.
  26. Provide supervision at all times by direct contact (In person), by vocera, or by phone during all hours services are provided.
  27. Provide direct supervision of holders of an IN temporary permit as defined by Code 844IAC6-1-2 to include a meeting with the actual presence of the supervising therapist or physician at least once each day. All documentation will be counter signed.
  28. Provide clinical supervision to Occupational Therapy and Occupational Therapy Assistant Students on site.
  29. Participate in and knowledgeable of QA&I activities.
  30. Ensure patient and departmental safety through work requests, following safety procedures and other appropriate measures.
  31. Supervise and direct O.T.A. and aides in provision of care. This includes patient scheduling and progression of treatments to optimize outcomes.
  32. Assume responsibility to see that new occupational therapy orders are promptly scheduled and notify Manager/Director when assistance is needed.
  33. Assist in orientation and training of new employees and students as appropriate.
  34. Conduct, participate in, and supervise the training and observation of medical and nursing personnel, students, and other persons in occupational therapy.
  35. Market and promote occupational therapy services appropriately.
  36. Assume responsibility for the direction of occupational therapy services as assigned by Director/Manager.
  37. Submit capital equipment recommendations as appropriate.
  38. Arrange for patient care items as appropriate. Responsible for maintaining appropriate supply inventory.
  39. Inform supervisor of department needs.
  40. Understand the significance of, and participates in, appropriate volunteer opportunities within the greater community promoting and supporting a higher quality of life for our region. These opportunities may include the enhancement of social, educational, spiritual, physical, or psychological needs within our service area.


Education/Experience:

Must be fully certified in Indiana to practice as an Occupational Therapist. May work with a Temporary Permit under the stipulated supervision of an O.T. in accordance with Indiana regulations.

Additionally, if assigned to provide services in Ohio, the O.T. is required to hold a current Ohio license.

Education: B. S., M.S., M.A., or OTD Degree or Certification in Occupational Therapy from an AOTA-accredited educational program.

Experience: none required. Preference may be given to individuals with advanced clinical experience or skills.

The O.T. must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients assigned, demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs, and to provide the care needed as described in department policies and procedures.

Successful completion of Reid Hospital general orientation, position-specific orientation skills checklist, and competency inventory required before working independently. Individual performance and competency-based skills are maintained through ongoing assessment of competence and educational activities. Written performance appraisals are completed at least annually.

Schedule Details:
Day Shift. 40 hours weekly. Monday, Wednesday and Thursday 7:30 am 6 pm, Friday 6:30 am 5 pm. Includes weekend/holiday rotation coverage on IP/ARU
