POSITION SUMMARY:

Under the supervision of the Director of Boston Allied Partners (BAP - LTSS Community Partner - CP), the CP Operations Supervisor (LOS) will provide administrative and operations support in preparing and completing all CP-assigned projects. Responsible for ensuring smooth functioning of project operations, including regular running and sharing of relevant reports with BAP staff and contracted ACOs and MCOs, preparation of spreadsheets, charts, correspondence, scheduling meetings, recording minutes, and managing calendars.

ESSENTIAL RESPONSIBILITIES / DUTIES:

The LOS performs a variety of duties in service of program operations optimization, including but not limited to the following:

  • Orientation of new staff to the program resources (e.g., OneDrive folders)

  • Rigorous program meeting scheduling of and note-taking (All-Staff, MassHealth, ACOs, Consumer Advisory Committee, Joint Operating Committee, Quality Management Committee, Ops Management Committee) Supervision of the Outreach Specialist Staff (Outreach Coordinators)

  • Regular operations data tracking and reporting work in eHana, reporting findings to the Director and Operations Management Committee. Reports include, but may not be limited to, the following:

  • Monthly Status and Outreach (MSO)

  • CP Completed Documentation

  • CP Missing Documentation

  • CP Care Plan Goals

  • CP Inpatient and ADT03 Discharge Follow-Up Activities

  • Other ACO-requested member updates

  • Submission of monthly and ad hoc assignment files from ACOs/MCOs Submission of monthly disenrollments in eHana

  • Management of Enrollee transfers and case escalations

  • Patient outreach via phone and email, as needed, in collaboration with the Outreach Coordinator

  • Maintaining patient databases Coordinate web-based teleconferences with multiple sites

  • Participating in the preparation of data and other reports for funders Maintaining patient information through the use of, but not limited to, Microsoft Excel and eHana (a cloud-based electronic health record system) Administrative tasks, including management of project records, handling of communications needs, maintenance of related files and records, and file-sharing with contracted entities (ACOs, MCOs, etc.)

  • Assisting in the development of reports, presentations and data analysis Consulting and collaborating with professional and academic experts in other institutional departments and community organizations, in areas related to project

  • Maintaining patient files in accordance with legal and institutional guidelines

  • Conforming to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care may be provided

  • Utilizing BMC values as the basis for decision making and facilitating the division's hospital mission

  • Following established hospital infection control and safety procedures Performing other duties as assigned or as necessary

JOB REQUIREMENTS

EDUCATION:

  • Associate's degree plus 3 years of relevant experience or

  • Bachelor's degree plus 1 year of relevant experience

EXPERIENCE:

  • At least one (1) year of supervisory experience preferred.

KNOWLEDGE, SKILLS & ABILITIES (KSA):

  • Strong computer proficiency: MS Office applications-Word, Excel, PowerPoint; Outlook; and web browsers

  • Excellent verbal and written communication skills

  • Self-motivated; excellent problem-solving skills; takes initiative to anticipate problems; seeks solutions proactively

  • Must be a dedicated team player, with a willingness to learn and to share knowledge with staff

  • Ability to work well with all levels of internal management and staff, as well as external clients

  • Experience working with multicultural and multilingual communities preferred.

  • Bilingual capability preferred
