Clinical Liaison/Care Coordinator

Summary: In compliance with all regulatory and licensing standards, manages and coordinates all
Inpatient rehabilitation for discharge planning and patient and family advocacy. Responsible for the
delivery of quality, cost effective services in rehabilitation care management. Responsible for
establishing yearly salary and non-salary budgets, establishing and monitoring policies and procedures
and for establishing performance improvement initiatives.
Education: Requires degree in Nursing, Rehabilitation or Social Work. Masters degree strongly preferred.
Licensure: Social Worker, Physical Therapist, Occupational Therapist or Registered Nurse.
Experience: Minimum three years experience.
Skills and Abilities:
Skills in organization and priority setting.
Demonstrated experience in responsive customer service.
Working knowledge/competence with national standards (JCAHO, CARF, Dept. of Health, Licensing regulations).
Ability to make high level strategic and complicated decisions and assume responsibility for such judgement.
Excellent skills in communication, collaboration and negotiation.
Management skills in planning, organizing, inventory control and directing others activities.
Leadership skills in motivating individuals and developing group cohesiveness
Skills in coaching, teaching, mentoring and problem solving.
