Minimum Job Qualifications:

Hold a valid Florida RN license. Boast a 2-year track record in a Skilled Nursing Facility. Exhibit profound understanding of MDS, Medicare, and care-planning.

DUTIES & RESPONSIBILITIES:

Assess and evaluate the quality of care being given to long-term care residents Perform initial interview and assess new residents Develop a care plan that has long-term and short-term goals for improvement Conduct meetings with multidisciplinary treatment teams, caretakers, and families Reach out to specialists within the community to get help in achieving care plan goals Attend departmental meetings as part of the Nursing management team and interact effectively with all levels of Nursing staff Conduct in-service education on MDS, RAPS, and Triggers Follow the MDS/Care Planning schedule for non-Medicare residents
