Job Summary
This position is for The Children's Center for Communicationand Development. The social worker will primarily provide on-site and remotesupport for the Center families and staff including needs assessments,referrals and recommendations, and consultations for specific issues thatimpede in the health and overall development of children enrolled in theprogram. Additionally, the position will provide service coordination through the state's early intervention program, First Steps, for children enrolled in the program with collaboration from the interdisciplinaryteam of specialists at the Center.

Primary Duties and Responsibilities
  1. Performsneeds assessments, develops plans and measures outcomes, makes appropriatereferrals and recommendations including follow-up for families and staff, meetingsocial work needs in accordance with the National Association of Social Workers(NASW).
  2. Consultswith families and the interdisciplinary team of therapists and educators, documentingcase updates and participating in weekly clinical meetings to shareinformation.
  3. Maintainand update a resource list for appropriate referrals with outside agenciesand organizations based on residency of enrolled families and familiesseeking services on the waitlist.
  4. Coordinates referral, evaluation/assessment, Individualized Family Service Plan (IFSP) development and maintenance, provision of services, and transition to school district, meeting all federal indicators for successful provision of services under Individuals with Disabilities Education Act (IDEA) Part C for children ages birth to 3 years.
  5. Participatein the waitlist management team, managing the organization and process of identifyinghigh priority cases in an organized manner, reaching out to families underthe supervision of the Director of Communications and Development.
  6. Performsother duties as assigned.


Minimum QualificationsMaster'sdegree from an accredited college or university in Social Work with licensure.Three to five years' experience in social work or clinical experience with aninterdisciplinary pediatric group working closely with families of childrenwith disabilities.

Knowledge, Skills & Abilities

  • Demonstrates the ability to create and maintain informationfor planning and programmatic needs using software that includes, but is notlimited to: Adobe Photoshop, Canva, Microsoft Word, and Microsoft PowerPoint,Microsoft Excel.
  • Ability to interpret and disseminate information from federal and state policies and code for the provision of service coordination and direct services under the IDEA law Part C.
  • Ability to remain empathetic and confidential when dealingwith a varied group of individuals and family needs, consideringrace/ethnicity, culture, socio-economic levels, and other potential causes forhealth disparities.


Preferred Qualifications

Backgroundwithin a pediatric interdisciplinary team.
