Salary: Starting at $48,000.00 (Non-exempt) About the Position: To organize, facilitate, and manage the coordination of care for referred youth involved with complex behavioral health concerns requiring multiple service providers. A Typical Day as a Care Coordinator: Organize a spectrum of effective, community-based services and supports for children and youth with or at risk for mental health of other challenges and their families that is organized into a coordinated network, builds meaningful partnerships with families and youth, and addresses cultural and linguistic needs, in order to help them to function better at home, in school, in the community, and throughout life. Cultivate and support provider and stakeholder relationships on identified cases with timely communication, inquiry, follow-up and integration of information into the care plan. Complete needed psychosocial and mental health assessments to determine treatment recommendations. Organize and facilitate Child and Family Team (CFT) meetings among providers involved with clients for a culture of respect, responsibility, and hope, utilizing a family-focused, systemic, strengths-based, and trauma-responsive model that are family and youth guided with the strengths and needs of the child and family determining the types and mix of services and supports provided. Assist the teams in developing and coordinating treatment and supportive services and in assessing treatment progress. Conduct community engagement and provides services to the child and family in home or other community-based locations. Maintain records and reporting in accordance with HFS requirements and/or Medicaid billing requirements. Does the Following Apply to You?
