St. Mary's Home care is seeking a Contingent Licensed Social Worker for a new Hospice agency in Bucks County, PA.
St. Mary's Home care is seeking a Contingent Licensed Social Worker for a new Hospice agency in Bucks County, PA.

St. Mary Home Care is a member of Trinity Health At Home, a national home care, palliative care and hospice organization serving communities in twelve states.

Our family-centered approach includes teaching patients and their families about their care, coordinating additional services, and researching community resources.

The Medical Social Worker is responsible for the provision of consultation and service to homebound individuals in the need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen.

Master's Degree from a school of social work accredited by the Council on Social Work Education. Currently licensed to practice as a Medical Social Worker as applicable for Pennsylvania.

Two (2) years of social work experience in health care setting.

Requirements:
  • Master's degree graduate of an approved clinical education program
  • Current clinical license in the state of Pennsylvania
  • Must have a minimum of two (2) years' experience of professional clinical experience in a home care setting,
  • Must have current Driver's license and reliable transportation to and from work site.
  • Ability to consistently demonstrate a commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program
