We are seeking an experienced and compassionate Nursing Care Coordinator to join our team at Trinity Health's Mercy Medical Center in Springfield, Massachusetts. In this role, you will work closely with our healthcare team to develop and implement individualized care plans for critically ill patients. Your primary responsibility will be to assist in creating and managing nursing care plans, including evaluating current and future medical needs of the patient, and participating in interdisciplinary team meetings to ensure comprehensive care. You will also communicate regularly with patients, families, and healthcare providers to ensure seamless coordination of care. To succeed in this position, you will need to have a strong background in nursing, with a focus on Critical Care. A graduate degree from an accredited college or university affiliated nursing program is required, and you must be currently licensed as a Registered Nurse in Massachusetts. Experience in Critical Care is preferred, and BLS (AHA/ARC) ACLS certification is required. We offer a competitive salary of $80,000 per year, based on industry standards and market conditions in Western Massachusetts.
