In this way, they build on the value of our brand promise to make healthcare easier and more accessible for those in need.Primary Roles and Responsibilities:The Pharmacist, Ambulatory Care is responsible for but not limited to the following (roles and responsibilities may evolve over time):Work as part of a clinical care team to improve patient satisfaction, lower medical costs, and improve quality of care metricsAs part of the clinical care team, you will participate in daily huddles with clinicians, medical assistants, and other healthcare professionals and help make recommendations based off reviews of patients' medications, testing results, and care gaps including accurate EHR documentationSupport patients and clinicians under a collaborative practice agreement to manage certain chronic conditions along with providing other patient care services and coordinating medication reconciliation across a spectrum of careSupport CVS to build out a clinical care model by providing continuous feedback and improvement opportunities for the care model, applying your knowledge to help differentiate CVS from competitorsDevelop other Pharmacists and Residents on best practices for operating within a clinical care modelPotentially, as capacity allows, support other CVS Pharmacy initiatives/tasks as neededRequired Qualifications:Active, unrestricted pharmacist license in the state of clinic location required1-3 years' prior relevant experience as a Clinical pharmacist -Ambulatory experience preferredBilingual in English and SpanishCompletion of all required training for current role within required timeframeAbility to stand and walk for an extended amount of timePreferred Qualifications:2+ years of clinical, ambulatory, or outpatient pharmacy practice experienceExperience working in ambulatory care setting or similar experienceExperience being part of clinical care teamBoard certification by the Board of Pharmacy Specialties (BPS) in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), or Geriatric (BCGP)Experience in primary care: internal medicine, geriatrics, or family medicineCompletion of a Pharmacy Postgraduate Residency (community or managed care PGY-1 and/or ambulatory care PGY-2)Experience with Medicare / Medicare Advantage plansImmunization certified or willingness to become certifiedPassion for teamwork and the opportunity to collaborate cross-functionallyEducation:Bachelors of Science or PharmD from an accredited college of pharmacyOur diverse work experience empowers colleagues for career success. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
