Bienvivir All-Inclusive Senior Health ("Bienvivir") is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly ("PACE").
PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.
BENEFITS for Full and Part-time employees who work 30 or more hours per week:
We pay 100% of the MEDICAL monthly premiums for Employee Only coverage.
We pay 100% of the DENTAL monthly premiums for Employee Only coverage.
We provide an affordable VISION monthly premium for Employee + Family coverage.
We pay 100% of BASIC LIFE for a benefit amount of $10,000.

Bienvivir is currently accepting applications for the following position:
HOME HEALTH RN PRN ON-CALL
The RN is primarily responsible performing routinely scheduled home visits and taking call during assigned shifts to dispatch to participants homes and conduct triage assessments. The RN will work under the direct supervision of the HH Nursing Supervisor / HH Director.
RESPONSIBILITIES:1. Be available via telephone to receive calls from BSHS telephone exchange and phone nurse triage line, to follow up with Participants medical needs.
2. Work in coordination with other after hours on-call staff via phone to address Participant medical needs and other concerns after hours.
3. Conduct participants' skilled care in their place of residence, working closely with PCPs, clinic, and HH staff.
4. Accepts referrals and medical orders and documents pertinent information required for initiation of services. Assists participant/family and physician in arranging for skilled services in the participant's place of residence.
5. Implements orders for medication and treatments as prescribed by the PCP's Plan of Care and BSHS policy/procedure.
6. Responsible for the completeness of Home Health records of participants, plan of treatment, progress notes, pertinent lab results, telephone orders, supervisory visits, team conferences, discharge summaries and medical updates.
7. Observes participant for changes in condition and reports changes appropriately to the PCP and/or designee. Obtains orders for changes in the medical plan of treatment to respond to participant's condition, documenting all observations and interventions in an accurate and timely way.
8. Initiates and applies appropriate preventive, therapeutic and rehabilitative procedures, and technique. Maintains communication regarding participant's care with the PCP, HH Nursing Supervisor, and family, as appropriate.
9. Assists and instructs participant, family members or other participant representatives in care plan implementation and other concepts and activities necessary to promote participant's health safety and independent living.
10. Accurately documents observations, interventions and evaluations pertaining to participant care management and services provided in accordance with Bienvivir Senior Health Services policy and procedures on the day services are rendered. Submits documentation in a timely manner.
11. Will work shifts to provide Participant visit coverage for the organization.
12. Participates in clinical competency testing for skilled nursing personnel.
13. Promotes the utilization of all available resources for the benefit of participants, families, and team members.
14. Documents electronically and provides feedback for team case conferences, to nursing staff and other BIENVIVIR disciplines as needed.
15. Participates in QA activities related to skilled services.
16. Assures proper documentation submitted in timely manner.
17. Reports all problem areas in SN component to the Home Health Nursing Supervisor / HH Director and conducts problem solving.
18. Other duties as assigned by Supervisor.
QUALIFICATIONS / REQUIREMENTS:1. Graduate from an approved professional accredited school of nursing, licensed to practice in the State of Texas.
2. Currently licensed (in good standing) through the State Board of Texas as a registered nurse.
3. Meets hiring criteria as defined in Bienvivir Senior Health Services' standards.
4. Have at least one year experience as an RN obtained within the last 36 months; or
5. Ability to meet Bienvivir Senior Health Services' conditions of employment regarding health clearance and provision of professional references.
6. Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with BIENVIVIR's participants and their families.
7. Must accept responsibility for maintaining clinical practice skills, learning, and adhering to Bienvivir Senior Health Services' policies and procedures.
8. Current CPR card.
9. Current TB test and/or chest X-ray.
10. Have effective telephone and interpersonal communication skills.
11. Experience in triaging the frail elderly in either a hospital, clinic, or home health setting
12. Bilingual in English and Spanish.
13. Must have automobile, valid driver license and current insurance.
14. Must be in good health and pass a physical upon employment if required.
15. Over 2 years' experience in Home Health and/ or PACE setting.
