Schedule: Monday - Friday, Flexible visit times
Key Responsibilities: Conduct thorough assessments to evaluate mobility, coordination, strength, and current level of function.
Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
Develop and implement written plans of care defining physical therapy needs, goals, progress, and expected outcomes.
Deliver high-quality physical therapy services in compliance with prescribed physician orders.
Maintain accurate and timely documentation of client performance, modifying goals and treatment as needed, and determining discharge care plans as appropriate.
Collaborate with healthcare team members, physicians, nurses, and family members to ensure seamless coordination of services.
Demonstrate effective communication and interpersonal skills, serving as a resource consultant for clinical staff.
Supervise other personnel as applicable, ensuring continuity of care and maintaining high standards of patient satisfaction.

Requirements: Degree in Physical Therapy from an accredited university/program.
Current, unrestricted state license as a Physical Therapist in the state of practice.

Experience in Home Health Therapy and completion of OASIS documentation.
