JOB SUMMARY

The Clinic Nurse (LVN or RN) provides patient centered care, education, and care coordination in a team-based multispecialty group under the direct supervision of the advanced practice registered nurse, physician assistant, or physician. The CMP Clinic Nurse works closely with various departments to deliver high quality care to patients and provide safe, effective nursing care in the clinic setting.

JOB DUTIES AND RESPONSIBILITIES
Job Specific:

  1. Interview patient to obtain vital signs and document information within the patient's electronic medical record. (EF)
  2. Clean and prepare treatment room for patient examinations. (EF)
  3. Follow clinic protocols and directions given by clinical team leaders or supervisor. (EF)
  4. Consistently implement communication, coordination, and cooperative efforts while serving as an essential member of the clinic. (EF)
  5. Work with members of the medical team to provide patient-centered care. (EF)
  6. Complete Medication Reconciliation for each patient. (EF)
  7. Coordinate scheduling for patients receiving tests outside the clinic. (EF)
  8. Facilitate provider authorization of prescription refills. (EF)
  9. Participate in performance improvement initiatives. (EF)
Under physician or APP orders:
  1. Perform routine diagnostic testing, including but not limited to, EKGs, audiograms, tympanograms, spirometry, and visual screenings with reporting and documentation as appropriate. (EF)
  2. Administer medications, vaccinations, or injections. (EF)
  3. Coordinate and schedule patients for care outside of the clinic setting (e.g., diagnostic testing, surgical procedures, home health, etc.). (EF)
  4. Counsel and educate patients regarding their care or treatment plan, including but not limited to, instructions for diagnostic testing or surgical procedures, completion of medical or consent forms, requirements for pre-test or pre-surgical clearance, holding or taking of medications, etc. (EF).
  5. Collaborate with hospital and clinic departments to ensure insurance coverage and authorization requirements are met in advance of services being provided. (EF)
Additional Duties:
  1. Accurately and completely perform and document patient follow-up activities including laboratory results and diagnostics, appointments, referrals, medications, and call backs. (EF)
  2. Keep all areas stocked and neat in appearance. (EF)
Hospital:
  1. Adheres to organization-wide and other applicable policies and procedures.
  2. Day-to-day performance complies with the hospital's Service Excellence Behavioral Expectations.
  3. Performs within the prescribed limits of the hospital's Ethics, Compliance and Confidentiality Program guidelines.
  4. Performs within professional Scope of Practice at all times.

Minimum Requirements

EDUCATIONAL REQUIREMENTS
Required: Graduate of an accredited school of nursing

Preferred: Additional education and/or training specific to clinic specialty.

EXPERIENCE

Preferred: At least one year experience in a physician's office or a clinic setting preferred.

SPECIAL SKILLS AND ABILITIES

Required: Demonstrate effective communication skills, must have the ability to multitask and prioritize duties in a fast-paced environment, ability to comprehend, recall, and to apply oral and/or written instructions. Proficient use of Microsoft Office and prior experience with electronic medical record platforms.

LICENSURE / CERTIFICATION REQUIREMENTS
Required: Currently licensed as a Licensed Vocational Nurse or Registered Nurse by the state of Texas; Healthcare Provider CPR/BLS
