Position SummaryThe Licensed Practical Nurse is responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Essential Duties and ResponsibilitiesThe essential functions include, but are not limited to the following:Provide skilled nursing care under the direct supervision of the clinical supervisor Complete evaluation tasks including vital signs and medication reviews Coordinate with physicians, therapists, and other individuals in the client's care plan Assess and chart observations of the patient's condition at each visit Maintain complete and accurate documentation of nursing care Work collaboratively with all members of the health care team Provide education to patients and families on proper home health care procedures and strategies Record patient's medical information and vital signs Monitor, record, and report symptoms or changes in patient's conditions Consult and coordinate with health care team members to assess, plan, implement, or evaluate patient care plans Administer local, inhalation, intravenous, or other anestheticsPrepare patients for and assist with examinations or treatments Modify patient treatment plans as indicated by patient's responses and conditions Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition Monitor all aspects of patient care, including diet and physical activity Assess the needs of individuals, families, or communities, including the assessment of the patient's home or work environments, to identify potential health or safety problems Engage in research activities related to nursing Perform other duties as assigned Minimum Qualifications (Knowledge, Skills, and Abilities)Associates degree required; Bachelor's degree from school of nursing preferred Current and valid Licensed Practical Nurse license One year of LPN experience Current CPR certification requiredValid driver's license requiredCurrent automobile registration and insurance required Strong interpersonal skills that build personal rapport and establish trust with the patientComputer literate Strong organizational skills Excellent written and communication skills Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers) Home health care experience Bilingual (Spanish) preferred Physical Demands and Work EnvironmentThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
