To learn more about our company, please visit Reports to: Departmental Lead, Supervisor or ManagerJob Type: Full-time position, 40 hours per week, Mon-Fri eight-hour shifts starting at 8am or 9am Job Location: Hybrid out of either of our locations: New Providence, NJ, Lenexa, KS or Stoughton, MA.Requirements High school diploma or GED. Bachelor's degree preferredPrevious experience working with insurance companies and healthcare professionals (physicians, nurses, etc.) is preferredPrevious experience with specialty pharmacy patient admissions and customer service in a similar referral source-based practice setting is strongly preferredIVIg case management experienced preferredMust have strong PC skills, especially the MS Office suite. Previous experience with the pharmacy order processing system, CPR+, is a major advantage (although training will be provided)Job Responsibilities, included but not limited to:Manage phone calls from agency partners for scheduling, rescheduling, other related agency activitiesCoordinate nursing contracts and authorizations when neededCollaborate with document management teams to obtain agency clinical documentationTriage incoming agency calls for field nurses and relay information to appropriate care team membersServe as communication liaison between pharmacy and nursing as appropriateIdentification and screening of nursing agenciesCompletes occurrence reports as applicableProvides care team members with feedback on assigned patients and gives suggestions for changes in procedure/protocolsAnswer phone, emails, and working in a variety of communication apps and electronic medical recores to provide clear documentation and coordination of servicesCommunicate with patients to coordinate delivery and nursing servicesScheduling appointments for patient visits with nursing personnel and agencies as neededLiaise and coordinate with nursing and pharmacy staff about schedules, patients, and any changesInput clinician visits and update in visit logs for new starts and adjusted schedulesResolve scheduling conflicts as they occurAttend staff meetings, teleconferences and patient care conferences as neededUnderstand and follow Promptcare's employee policies and proceduresPerform other duties as assigned and is able to work longer than normal hours with little or no notice, as neededPhysical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and listen. The employee is required to occasionally walk, and bend or stand as necessary.
