Hourly Pay Rate: $40 Hourly (W-2), $45 Hourly (1099)

Physical Therapist Job Duties:
•Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
•Supervise PTAs daily.
•Provide quality therapy services to individuals in an educational setting
•Monitor and document patient progress

Physical Therapists employed by Flagstar Rehab will receive:
•up to 23 days PTO

Physical Therapist Requirements:
•Current PA PT License and Registration
•CPR Certification
•EMR Competence

Meets all State and Federal requirements for US employment in a Nursing Facility
•Malpractice/Liability Insurance (if applicable)
