The Mary T Family of Companies is seeking at Physical Therapist to work with our Home Health clients This position will be based in Coon Rapids, Minnesota and serve clients in the nearby metro area, as well as clients in the Cambridge area. The Physical Therapist is responsible for evaluating needs of referred patients, formulating treatment plans in home care environment, and providing physical therapy services defined in treatment plans. The position works cooperatively with RN case managers, physicians and other home health staff and supervises the physical therapy assistant staff.SCHEDULE: Part-time or Full-time hours available, but full-time preferred, 30-40 hours per week. The schedule can be flexible and set up by the employee. Visits will take place during daytime hours (8a-4p), Monday - Friday. No weekends required ESSENTIAL FUNCTIONS AND BASIC JOB DUTIESExamine and evaluate individual patients and develop a treatment plan to improve mobility, reduce pain, restore function and prevent disability.Establish appropriate treatment goals of individual patients.Implement physical therapy Point of Care by administering exercises, training in use of assistive devises, instructing, encouraging, and assisting patients in performing physical activities to improve ambulation, functional activities and actives of daily living.Document patient care by charting in patient's record.Supervise physical therapy assistants.Confer with the patient, medical practitioners, and appropriate others to plan, implement, and access the intervention program.BASIC QUALIFICATIONS FOR THIS POSITION:EDUCATION:Master's Degree in Physical TherapyCERTIFICATION/LICENSURE:Current Physical Therapy License in MinnesotaValid driver's licenseEXPERIENCE:Minimum one (1) year experience in home health, a short-term rehab, outpatient, or hospital inpatient rehab setting.KNOWLEDGE, SKILLS, ABILITIES:Strong communication, organizational, collaborative, and prioritization skillsAbility to build and maintain trusting relationships with patients, employees, and outside entities.Ethics and integrity with advanced problem solving skillsDemonstrated knowledge and skills necessary to provide care and communicate with primarily the geriatric populationDemonstrated knowledge of the principles of growth and development over the life span.Able to assess data reflecting the patient's status and interpret the appropriate information needed to identify each patient's requirements to his or her age-specific needs.Basic computer and math skillsLeadership skills with ability to coach and train staffKnowledge of and ability to practice safe patient handlingWHAT WE HAVE TO OFFER YOU: $45 - $51/hrPaid Time OffPaid holidaysInsurance benefits including Medical, Dental, Vision, Voluntary Life, HSA and more401(k) with MatchAnnual Merit IncreasesDaily Pay Available: Work today, get paid todayProfessional Development Scholarships and OpportunitiesEmployee Referral Bonus Program About Us: Family owned and founded on a history of care, The Mary T Family of Companies provides a full continuum of health care services from disability services, senior housing, assisted living, therapy, home health, and hospice care.
