Job Title: PharmacistJob DescriptionWe are seeking a dedicated Pharmacist to perform comprehensive medication therapy reviews with eligible members. The pharmacist will review members' medication profiles, identify, prevent, and resolve medication-related problems to help members maximize benefits and minimize costs. Additionally, the role involves conducting encounters with members to improve medication adherence, addressing gaps in therapy, documenting these encounters, and collaborating with other healthcare professionals. The pharmacist will also contribute to various projects to support business needs and provide assistance and guidance to team members in MTM/CCM support roles.ResponsibilitiesPerform comprehensive medication therapy reviews with eligible members.Review members' medication profiles during sessions to identify, prevent, and resolve medication-related problems.Document member encounters in the MTM platform and complete outreach to other healthcare professionals as necessary.Conduct encounters with members to improve outcomes related to medication adherence, gaps in therapy, and other measures.Contribute to projects as needed to support business needs.Collaborate with team members in MTM/CCM support roles by providing assistance and guidance.Perform other duties as assigned.Essential SkillsDoctor of Pharmacy (PharmD) or Bachelor of Science Pharmacy Degree.Current pharmacist license in good standing with the State Board of Pharmacy in the state where the position is located.Eligible to work in the United States without need for work visa or residency sponsorship.Prior MTM/ambulatory care experience required.Minimum of 1 year of MTM-specific experience.Additional Skills & QualificationsSpanish-speaking candidates preferred, not required.Preference for insurance/PBM pharmacists over hospital/clinical pharmacists.Preference for candidates in CST/EST time zones; will consider candidates in MST.Proficiency in MS Office products.Excellent communication skills to build rapport quickly with patients and show empathy over the phone.Ability to collaborate and support leadership initiatives within the larger overall team.Adaptable and able to reprioritize quickly in a changing environment.Work EnvironmentThis is a fully remote position. Standard 1st shift, M-F work hours.
