POSITION: HEALTH NAVIGATOR/6-12 NURSE

DURATION: Full Time (10-month school year)

DUTIES AND RESPONSIBILITIES:

Directly responsible to the School Superintendent/K-12 Principal.

Coordinate and monitor all aspects of health related student services throughout the K-12 system. (mental/behavioral health, AODA, appointments/assessments, school wide health screenings for vision, dental, hearing, concussionprotocols, athletic physicals).

Works in collaboration with the primary care providers and all members of theschool health/counseling team, including parents and families.

Assesses healthcare needs and identifiescommunityresources.

Coordinate health related community resources, screenings, service delivery within the school setting.

Serves as student advocate and assists in identification and improvement of service delivery.

Provide individual patient/family education and self-management support that is appropriate based on language, cognitive abilities, literacy level, learning style, cultural norms, patient preference, readiness for change and resources available.

Address medication adherence.

Collaborate in school wide goal planning and care management.

Assist students in improved healthcare access; act as advocate for students within the school setting.

Serve as a clinical resource and community referral resource within the School.

Assist providers with shared medical appointments and group visits.

Participate in regular staff and school wide trainings.

Document communication in electronic student records system, uploading immunization records.

Willing to assist with other school activities as requested or assigned.

Assist in crisis response, school safety and school wellness teams.

Willing to assist with other school functions, such as, but not limited to: School Feasts, Powwows, and Graduations.

Other duties as assigned.

QUALIFICATIONS:

Minimum Bachelor’s Degree in Human Services or related field preferred.

Two years of nursing, social work, counseling or therapeutic work and experience in community Preferred.

Strong written, oral and communication skills.

Experience working with children and families in schools or communities preferred.

CPR Certification required.

Experience with utilization review and discharge planning required.

Must be supportive of the philosophy, concept, policy and procedures of the Lac Courte Oreilles Ojibwe School-Waadookodaading.

Native American preference in employment.
