Job Description Metro is looking for a motivated and passionate PER DIEM Physical Therapist to provide in-home care. Come be a part of a growing organization in a progressive, supportive, and innovative team All levels of experience are encouraged to apply and will train the right new graduate. * Job Type: Per Diem * Pay: $65-$80 per visit * Location: In and around White Plains * Schedule: Flexible What We Offer * 1099 and W-2 options to fit your preferred work style * Competitive salary * Longstanding Success * Opportunities for professional growth and development * A professional, supportive and fun work environment fostered by a team of well-respected clinicians who take pride in the challenges of meaningful work * Focus on Patient Care - Quick EMR (No Oasis required) * Great work/life balance * Flexibility and Autonomy - see patients any 7 days of the week * Preference given to see patients in your primary geographic area Responsibilities: * Accept and treat patients within your designated geographic area * Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans * Develop a plan of care and treatment goals in consultation with the physician, patient, and family * Plan, prepare and implement customized therapy programs to restore function and promote independence * Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques * Provide timely and accurate documentation for each visit * Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning * Deliver care in compliance with agency policies and professional practice standards Qualifications * Physical Therapist Program Graduate * Licensed Physical Therapist in the state of New York or eligible. * Mandatory Certifications: First Aid, AED, & CPR Certifications. * Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance. * Ability to travel to patients' homes within the assigned geographic territory Additional Information
