He or she is responsible for evaluating and monitoring drug therapy relative to the total needs/problems of the patient while collaborating with physicians, nurses, and other healthcare professionals in an effort to achieve the best possible outcomes.Schedule: Monday - Thursday, 8:00AM - 5:00PM, rotating on call responsibilities. Minimum Qualifications:Experience/Education:Bachelor of PharmacyActive pharmacist licenseClinical Pharmacy experience from work or clinical rotation experienceProficient at partner programs (i.e.: MS Outlook, Word, Excel).Ability to cope with ambiguous, changing environment while under pressure in a calm manner.Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion/AccuRX team.Essential Functions:Reviews, receives, and verifies all patient orders and prescriptions and documents according to policy and procedure and state and federal pharmacy law Participates in the direct supervision of Pharmacy Technicians, Pharmacy Support Technicians, and Pharmacy Logistics Technicians Participates in on-call rotation for after-hours availability for either home infusion or ambulatory infusion Maintains proper competencies based on assigned role Maintains active licensure and current on all required continuing education Acts as a drug information source for internal staff and outside referral sources Provides comprehensive patient counseling to patients who accept and request a pharmacy consult Reviews clinical information including applicable labs and patient information for the safe provision of medication Adheres to all federal and state pharmacy laws and regulations Clarifies and receives orders from a physician or medical agentAbout the Benefits:Competitive Compensation401K Retirement PlanEmployee Assistance Program (EAP)Employee DiscountsRamsey SmartDollar ProgramReferral ProgramPaid Time Off (Full-time/Part-Time)8 Paid Company HolidaysPhysical Demands:The physical demands described here are representative of those that must be met by an employee to successfully perform the primary functions of this job.
