Job Overview This is a union position and a part-time position. HealthRIGHT 360, a family of programs, is located across 13 different counties in California. HealthRIGHT 360 provides an array of medical and behavioral health services to some of the most vulnerable populations, ranging from primary care, mental health services, detox, substance use disorder treatment in both residential and outpatient settings, and transitional services for the criminal justice population. As a Care Coordinator for MAAC (Multi-Agency Assessment Center), your primary task will be substance use services onsite at the Santa Clara County Juvenile Hall for incarcerated youth who are in need of support in order to make healthier choices and are motivated to make positive changes in their lives. By engaging youth in group workshops and individual services, you will help them develop skills that can assist participants in learning the effects of substance use and coping with emotional issues in a healthier way. You will also have the benefit of working with a team of enthusiastic, experienced professionals who will provide clinical supervision to help you grow and will work with you to create a flexible schedule that works for your life. The Care Coordinator will deliver and coordinate services focused on Substance Use for adolescents and Transitional Aged Youth residing in Santa Clara County Juvenile Hall. The Care Coordinator works directly with clients under the supervision of the AARS Youth Services Supervising Care Coordinator and Director of Outpatient Services. Key Responsibilities Facilitate educational workshops to assigned boys/girls units. Conduct initial screenings and individual sessions. Conduct comprehensive, ongoing assessment of client’s substance abuse and mental health needs. Work with clients to develop and update individualized treatment plans with specific goals and objectives. Conduct case management and individual counseling sessions as needed. Coordinate social/recreational/community service activities including outreach events and rallies. Provide referrals and linkage to AARS services and/or other community-based organizations. Establish and maintain liaison with the Probation Department and community organizations. Attend Probation Open Houses, Parent Resource Fairs, or other Juvenile Hall events which may occur on a weekend. Provide and/or assist with arranging translation and other practical support services. Maintain confidential client records, files, and prepare data/narratives for monthly reports. Provide necessary services at Santa Clara County’s Juvenile Hall and AARS agency site as appropriate. Attend all mandatory meetings and trainings assigned by the program manager, clinical supervisor, or Director of Outpatient Services. Other duties as assigned by the program manager. Must be able to work weekday afternoons/evenings or as needed. Education and Knowledge, Skills and Abilities Bachelor’s degree in Counseling, Psychology, Social Work or related field AND Registered with California Certification Board of Alcohol and Drug Counselors (CCBADC). Master’s degree AND registration with the Board of Behavioral Services (BBS) preferred. Familiarity with theoretical orientations (e.g., motivational interviewing, client-directed outcomes, CBT, Familiarity of Seven Challenges and Seeking Safety models). Minimum of one year experience providing health or human services for adolescents, including substance abuse recovery services. Knowledge of cultural stigmas of counseling and treatment services. Knowledge of principles and techniques of social work, case management, crisis intervention, client-centered counseling, and group facilitation. Experience working with co-occurring disorders. Understanding of adolescent developmental issues. Familiarity with the juvenile justice system. Experience working with gang-involved youth and families. Ability to maintain a flexible work schedule; some evenings required. Ability to utilize supervision and build on skills critical to the job functions. Ability to organize, schedule, and efficiently use time to complete all duties and responsibilities assigned. First Aid certified within 30 days of employment. CPR certified within 30 days of employment. Bilingual/Bi-cultural in Spanish highly preferred.
