Children's HCBS Supervisor
Monsey, NY
$40.00+ per hour
The role of the HCBS Supervisor is to oversee the delivery of home and community-based services to children/youths and their families. The HCBS supervisor coordinates service plans, leads a team of HCBS direct service providers, and collaborates with other care team members.
Responsibilities:
- Assessments & Service Planning
- Conduct discovery meeting with participant/family and care team for new referrals.
- Conduct service plan meetings with participant/family and care team for those renewing services after authorization period ends.
- Review participant's medical/behavioral records and collect additional medical justification.
- Conduct a health and safety assessment.
- Establish SMART goals based on participant's input.
- Determine Frequency/Scope/Duration of services needed as per the set goals.
- Write up all components of the Service Plan(s) and additional required documents.
- Ongoing review of participants' service plan goals to ensure adequate progress is obtained.
- Supervision & Leadership
- Hire HCBS Providers and train them regarding each child/youth's Service Plan, as often as needed.
- Supervise and guide the HCBS Providers with ongoing support, coaching, and check- ins.
- Provide leadership to HCBS Providers and assist with challenges related to service delivery. Review and approve each session progress note and provide feedback to the HCBS Provider.
- Service Oversight
- Keep track of each participant's progress/barriers to service delivery, including the utilization of their approved hours
- Conduct regular visits to the service delivery locations.
- Communicate regularly with participant/family and other care team members (medical professionals, therapists, care manager, etc.)
- Maintain detailed and current case notes for each participant.
- Collect and analyze statistics on overall service utilization, outcomes, and other measurable data.
- Continuously assess service delivery to ensure it meets quality standards and the State's guidelines.
- Strong leadership and organizational skills.
- Excellent writing and verbal communication skills
- Computer/technology proficiency
- Valid driver's license and reliable transportation.
Credentials:
CFASS Supervisor: Master's degree with one year of experience or a Bachelor's degree with four years of experience in human services working with children/youth.
