Program Description The Flexible Housing Subsidy Program offers an exciting opportunity to participate in a program to make a difference in the lives of people who have been living on the streets in San Francisco. During the COVID-19 pandemic, many unhoused people sheltered in place (SIP) in local hotels throughout the city. Now, as the pandemic has shifted to a new phase of crisis management, unhoused people in shelters and on the street are being offered a permanent housing subsidy with a 30% contribution of monthly income at apartment buildings and homes throughout the city. The Case Manager will be part of a multidisciplinary team with community specialists, case managers, and clinical case managers. This team collaborates with community partners in crafting successful strategies to support people as they transition to life in new homes across the city.

The Flexible Housing Subsidy Program serves adults and older adults transitioning into permanent housing by providing comprehensive and integrated services, including harm reduction and recovery-oriented treatment, physical and mental health case, housing assistance, and vocational and educational support. We work in partnership with San Franciscos Department of Homelessness and Supportive Housing (HSH) and Brilliant Corners. Our team is deeply recovery-oriented, and we pride ourselves on meeting people where they are at. Job Description Our case managers, as members of our multidisciplinary treatment team, provide coordinated care services for approximately 20 residents transitioning from homelessness to permanent housing. The Case Manager (CM) will provide case management support to adults and older adults who may be homeless, isolated, and living with mental illness, cognitive impairments, and/or substance abuse. Residents may have other needs, as well (re-connecting to family, benefits check-ups, getting primary care set up, finding a venue for developing a love of painting or poetry, or becoming a peer leader and homeless outreach worker, to name only a few examples). Case Manager duties include undertaking needs assessments, developing care plans, providing crisis intervention, emotional mental rehabilitation and wellness work, coordinating care among service providers, providing monthly wellness check-ups and annual comprehensive reassessments, and, for those residents who thrive in their new permanent homes, discharge planning and thoughtful, recovery-oriented termination. Team meetings occur collaboratively at least once a week; the CM will also meet with a program supervisor weekly. The CM will also meet as needed with other case managers in the city for case review and consultation and will participate in monthly trainings within the agency as indicated. Additional responsibilities include advocating with outside agencies on behalf of clients to secure benefits, housing and resources, and maintaining a professional working relationship with providers and other support systems. This position reports directly to the Flexible Subsidy Housing Program Manager. Program Specific Responsibilities: Provide case management services, including evaluations, care plan development, and assisting clients in accessing housing, financial, medical and other needed services. Complete all initial housing assessments within 72 hours of client contact. Provide crisis intervention and mental health support as needed. Assist with linkage to housing resources, benefits, medical services, and other needed services. Work collaboratively with collateral agencies and housing providers, including Housing Coordinators from Brilliant Corners and landlords. Advocate on behalf of residents. Collaboratively problem solve around barriers with residents, providers, and other housing supports. Maintain documentation standards with progress notes, assessments, and plans of care. Maintain thorough clinical records after each contact or service, in accordance with standards set by the Felton Institute, SF Homeless and Supportive Housing, Medi-Cal, and program funders. Participate in team meetings and care conferences. Additional duties as needed will be assigned. Minimum Qualifications: Passionate about working with unhoused adults and older adults. Highly interested in the principles of recovery and dedicated to creating caring, supportive environments. Willingness to work from a strengths-based, harm reduction, and person-centered model, with an emphasis on evidence-based practices. Accurate and detail-oriented in documentation of plans of care and charting. Experience in case management and mental health services for people experiencing homelessness, and for those living with mental illness, a plus. Experience serving older adults not required but extremely helpful. Ability to work with diverse cultures, ethnicities, and gender/sexual identities required. Strong organizational, communication, interpersonal and time management skills preferred.
